| Do you use the Quality Variance Report (QVR) System? |

It has come to my attention that some members of our
medical staff may hesitate to complete a Quality
Variance Report (QVR). Their reasons are many and
varied. The fact remains, though, that there may be
incidents which a doctor should report in order to
assure process improvement and/or staff
development.
In my role as Chief of Staff, I
have made every effort to personally meet the needs of
our medical staff members. Once again, therefore, I
would like to offer my cell phone number (619-823-
1497) and encourage you to call me whenever you
have a concern. Your comments to me will be held in
complete confidence and your anonymity will be
respected. You can be assured that I will take
whatever steps necessary to address your
issues.
