Informed consents must be provided to patients or family members prior to the surgery or procedure occurring.

The attestation must be documented in the medical record either in the history & physical or the progress note, prior to the surgery or procedure.

The physician may also document on the Physician Verification Form that informed consent was provided prior to surgery or procedure occurring.

The documentation must indicate that the procedure's risks, benefits and options have been discussed.

Press Ganey, an independent survey organization, is conducting Sharp Grossmont Hospital's Physician Satisfaction Survey. They have selected a random population of the SGH medical staff.

To date only 16% of the SGH physicians who received the survey have completed it.

The input that the members of our medical staff provide in the physician satisfaction survey process is critical to the hospital's strategic planning process.

Please check to see if your office has received a survey from "Press Ganey." If you received a survey, but have not yet returned it, please plan to do so without further delay.
