    California Department of Public Health
    Senate Bill 739

    Effective July 1, 2008 California Department of Public Health (CDPH) now mandates and will enforce compliance with the following procedures:

    Surgical Care Improvement Project (SCIP)
    -Prophylactic antibiotic received within 1 hr prior to surgical incision
    -Appropriate prophylactic antibiotic selection for surgical patients
    -Prophylactic antibiotics discontinued within 24 hr after surgery end time (48 hr for cardiac patients)
    -CDPH will download this data directly from the CMS Hospital Compare website


    Central Line Insertion Practice (CLIP)
    -The process measures must be reported to National Healthcare Safety Network (NHSN) for all ICUs: adult, pediatric and NICUs within the facility.

  • Reappointment Process - Late Fees
  • To encourage all members of the medical staff to submit their completed applications for reappointment in a timely manner, based on a recommendation from the Credentials Committee, the Medical Executive Committee approved a policy entitled, "Late or Incomplete Medical Staff Reappointment Application Fees."

    There will continue to be no fee for completed reappointment applications received within the first 45 days. However, applications received after that time will be subject to a progressive fee schedule (46-90 days - $100 and 91 to 120 days - $200). No reappointment applications will be accepted after 120 days and the physician will be required to reapply for staff privileges and to pay a new application fee.

    The policy also clearly defines the following components of a complete reappointment application:

    -Completed and signed application form
    -Completed and signed privilege form
    -Completed and signed Code of Conduct
    -Completed TB Screening Form
    -Completed addendum re: Covering Physician
