ROMACC Update |

The new meds are often the ones that have
been
missed on the Home Med List at Discharge.
Please:
Use the "NEW MED LINES" at the bottom of the Home
Med List Admission to help with ROMACC at time of
discharge.
Write the new medications that have been started and
on which the patient needs to go home - this is NOT a
prescription.
The nurse has in ONE location
a complete discharge
medication list that can be used to document the
Home Med List at Discharge for both the patient and
the
medical record.
To dictate the discharge summary, see the
information under the
ROMACC tab in the chart.

Patient Satisfaction "Flame" |
|
Effective March 1, 2008, physicians and staff may see
a small (2 x 3 inch) magnetic flame on the doorframes
of patients' rooms. The flame is a reminder that the
patient has been inconvenienced at some point during
their hospital stay. SGH staff have been trained to
assure that patient that everyone at SGH is committed
to making sure the rest of their stay goes beyond
their expectations.
