To ensure your participation in the HealthPartners Workers' Compensation network when it transitions to ChiroCare on September 1, 2010, ChiroCare must receive your signed Addendum (signature page only) by July 16, 2010. Signed signature pages may be submitted via fax or mail.

Materials received after July 16, 2010 will be processed but may be subject to a later effective date.
Upon receipt of a signed Addendum, ChiroCare will return a fully executed copy to your office via mail.
Please note that HealthPartners will be updating its on line Workers' Compensation Provider Directory following the September 1, 2010 transition. Because this directory is updated only once a year, ChiroCare providers who do not respond by July 16, 2010, and are not contracted as of September 1, 2010, may not be included in this updated version.
