While registration for the September 23rd seminar will be accepted while space is still available, the early registration numbers will be used to determine how much space is reserved by ChiroCare. To ensure you have a seat at this exclusive event, please submit your RSVP via the tear off card included in the brochure previously mailed, or by submitting an email to info@chirocaremn.org today. Emails should include the name of the provider, a contact phone number and if applicable, the name of a staff member who will also attend.

David Moen, M.D.; Executive Medical Director of Care Model Innovation and Network Development, Fairview Health Services
David Moen, M.D., leads health care innovation for Fairview Health Services, the second largest health system in Minnesota. Dr. Moen develops collaborative relationships with stakeholders across the health care industry to establish Fairview as a national innovation leader. In addition, Dr. Moen serves as the Chief Medical Officer of NetClinic, a web-based program that serves as an interactive personal health portal to allow virtual care delivery involving patients and clinicians. He has also held various leadership roles including Chief of Medical Staff for Fairview Lakes Regional Medical Center, member of the Executive Committee of the Fairview Lakes Health Services Boards of Directions, member of the Board of Directors for Lakes Region Ambulance Service and board member for the Minnesota College of Emergency Physicians.
Michael Bromelkamp, CPA, MBA, CIA; Principal, Olsen Thielen
Mike has more than 34 years of experience in accounting, auditing and consulting. He joined Olsen Thielen in 1992 and has an MBA from Carlson School of Management. His experience includes managing corporate finance, accounting and internal audit functions for national public and private companies. Mike works with small and mid-sized companies and non-profit organizations to maximize their financial and operating results. He also Chairs the Health Care Policy Committee at the Minnesota Chamber of Commerce.
Jason Mihalakis; President and Executive Creative Director, catchfire
Jason is a market-savvy creative professional with a 15 year track record of results-oriented advertising and marketing efforts for clients such as 3M, Best Buy, Ceridian, General Mills, Hallmark, Medica, Nabisco, Procter & Gamble and United Healthcare. Jason and his catchfire team have been working with ChiroCare to develop new identity, positioning and communication initiatives designed to increase the number of patients obtaining care through ChiroCare's provider network.
Terry Crowson, MD; HealthPartners Medical Director
Dr. Crowson was named the 2008 Leader in Disease Management by Managed Healthcare Executive magazine and is currently the Medical Director for Medical Management and Government Programs at HealthPartners. Dr. Crowson's team works to support disease management programs, medical policy, pharmacy service and government programs. He has been with HealthPartners since 1975.
