"Dear Colleagues,
I am very pleased to inform you that the Federal Trade Commission has issued a 90-day delay in the enforcement of its Red Flags Rule, which would have gone into effect May 1. This delay will give the ADA more time to challenge its applicability to small health care providers such as dentists. "

Comparing Dentist Anesthesiologists With Medical Anesthesiologists in the California Dental Office

Recently some physician anesthesiologists new to dental anesthesia have been handing out literature making some pretty wild claims.

Some of these claims question the competency and qualifications of the dentist anesthesiologist, implying we are not safe, and that only a medical board certified MD anesthesiologist is qualified to provide anesthesia in the dental office.

While MDs in the hospital are my first choice for open heart surgery, they are near my last choice patients having dental surgery.  It is vital that for any anesthesiologist to provide the best service they must thoroughly understand the surgical procedure, how it affects the patient, how long each procedure will take and when the patient needs to be deep or light.  Outpatient ambulatory anesthesia care for dental procedures actually requires more foresight, preparation and attention BECAUSE THE SURGERY IS TAKING PLACE IN THE PATIENT'S AIRWAY.

Since I am one of only a very few dentists who completed a medical Residency in Anesthesiology, I am uniquely qualified to know what physicians know about dental anesthesia. Frankly, it is near zero. Physicians flooding into the dental offices seeking work they can't find in hospitals are therefore on a steep learning curve for anesthesia for dental surgery.  It took me several years to blend my degree in  dentistry with my residency experience providing anesthesia for open heart surgery, neurosurgery, etc.,  into a safe outpatient anesthesia technique for dental patients.  It is not a natural crossover.  The physician based anesthesiologist is used to staff backup and equipment supplied by the hospital. 

The California Dental Board wisely recognizes the complex issues regarding outpatient dental anesthesia, and is precisely why the  Board requires an in-office evaluation of equipment, anesthesia technique and knowledge of medical emergencies.  As one of those examiners for the Dental Board I have been told that even after 6 years perhaps 50% of physicians with the MGA permit still haven't been evaluated by the Board as to their competency.  I am working with the Board to resolve this potentially dangerous situation.

Anyone utilizing the services of an anesthesiologist has the right to know if they have, in fact, been evaluated, not just have "a permit".
