Osteochondral FragmentHistory: 
Fourteen year old Quarter Horse gelding that presented to Animal Imaging for a full bone scan.  The horse was injured in a barrel racing performance two weeks prior to presentation when a left hind limb became intangled in a panel.  The horse showed signs of lameness soon thereafter, which was attributed to the pelvic limbs (primarily the left stifle).
Osteochondral FragmentRadiographic/Scintigraphic Findings:  Evidence of an osteochondral fragment associated with the tibial tuberosity along with intracapsular swelling is noted.  This is located in the region of attachment of the cranial cruciate ligament and concurrent trauma to the cruciate ligament could be considered.  In addition, there is decreased density involving the remainder of the proximal tibial tuberosity with a rim of sclerosis.  This is consistent with osteomalacia.  This appearance is also suggestive of concurrent osteomyelitis.  The intense focal scintigraphic uptake along the entire tibial plateau supports the possibility of osteomalacia of the proximal tibia.
Osteochondral Fragment 
Impressions and Recommendations:
Arthrocentesis is recommended to evaluate the character of the synovial fluid and rule out concurrent septic arthritis.  Arthroscopic evaluation and lavage may be considered.  Ultrasound evaluation of the joint may also be indicated.
