History: A 4 year old Quarter Horse gelding
presented to the referring veterinarian with a Grade
III lameness in the left hind. Regional anesthesia at
the level of mid MT-III alleviated the lameness.
Radiographs were unremarkable. Intra-articular
anesthesia of the fetlock did not change the
lamness. An MRI exam was requested of the left
hind foot and fetlock.
Findings: An exam revealed severe "bone
edema" in the distal cannon bone of the left hind
leg. The bone edema represents an early stress
fracture or an impending stress fracture (condylar
fracture).
Impressions and Recommendations:
Bone edema as visualized on a fat
suppressed STIR sequence or as a dark area on an
SPGR study is known to be quite painful. It will,
however resolve with adequate rest. A recent
followup exam on a similar case was sound in four
months with normal MRI findings.
