Age Well, Live Well

 

 

Age Well - Live Well is an initiative of the Texas Department of Aging and Disability Services (DADS) Volunteer and Community Engagement (VCE) office that strives to educate and motivate individuals to prepare for the future by encouraging healthy lifestyle activities, building awareness of available resources, and creating change through volunteerism and community engagement activities. click here

Partnering Physicians with Community Organizations is a toolkit developed by The National Council on Aging Center for Healthy Aging and the Merck Institute of Aging & Health designed to help physicians collaborate with community-based organizations that serve older adults.  It also includes many suggestions and tools useful to organizations collaborating with physicians, including tips on working with physicians, a referral form template, and a sample surveyClick here for more information.

Texas STAR Program

Your Health Plan - Your Choice 

 

STAR is a Texas Medicaid managed care program and will include persons living in Gregg, Harrison, Marion, Panola, Rusk and Upshur, as well as other surrounding counties.  In STAR, you will pick a health plan for your health care services.  You will also pick a main doctor (a primary care provider).  The Star program will now be implemented in the North East Texas Medicaid Rural Service Area on March 1, 2012 with enrollment beginning now.  Please click here for information regarding a local presentation where representatives will be happy to provide you with all the information that you need to choose the plan thats right for you.  It is very important that you choose a managed care plan BEFORE February 10, because if you do not, the State will auto assign a plan to you along with a primary care physician.  Watch your mail for your enrollment packet and call 1-800-964-2777 if you have any questions.
