A new report issued by Department of Health and Human Services Agency for Healthcare Research and Quality (AHRQ) shows that 86 percent of nursing home staff participating in a survey on safety culture feel that residents are well cared for and safe. The report also shows that just over half of staff (51 percent) reported that they feel safe reporting mistakes. The first annual

Nursing Home Survey on Patient Safety Culture: 2011 User Comparative Database Report

analyzes data from 16,155 staff within 226 US nursing homes. Nursing homes that have administered the Nursing Home Survey on Patient Safety Culture can use the comparative database report to compare data with other nursing homes; assess their strengths and areas for potential improvement in patient safety culture.   NEW TRANSPORTATION FUNDING OPPORTUNITY

 

The Veterans Transportation and Community Living Initiative (VTCLI) notice of funding ($30 million) is a partnership between the Departments of Defense, Health and Human services, Labor, Transportation, and Veterans Affairs (VA).  Applicants must be eligible to receive Federal Transit Administration (FTA)  �5309 funding (i.e. public agencies such as transit authority, city/county government, metropolitan planning organizations, or the State).  Partnerships with the Aging and Disability Networks are encouraged.  The existing VA relationships can facilitate meeting the requirements of this Notices of Funds Availability (NOFA).  The United We Ride Ambassador for your region may be available to assist with the NOFA planning process.  The application deadline is September 16, 2011.  For more information on this funding opportunity, click here

 

 

 
NEW ADMINISTRATION ON AGING CARE TRANSITIONS TOOLKITS 
 
  The New AoA Care Transitions Toolkit  is targeted to organizations interested in learning more about how to prepare for a role in care transitions programs. Whether your organization has historically developed partnerships with health care providers or programs that span the continuum of care such as Community Living, Money Follows the Person, Aging and Disability Resource Centers, or others, the tools and resources here can assist in formalizing your efforts for future funding and program opportunities.

 

This toolkit includes lessons learned from States that received funding from the Aging and Disability Resource Center (ADRC) Program, ADRC Evidence-Based Care Transitions Program, Person-Centered Hospital Discharge Model, Community Living Program (CLP), and the Veteran Directed Home and Community Based Services (VDHCBS) Program.  click here for more information.
