Medical Quality 2011: A Great Success!

As your newly elected president, it is my first responsibility to update the college on our successful meeting in beautiful San Antonio, Texas- Transformative Changes in Healthcare: A Roadmap to Improve Quality, Efficiency and Outcomes. For those that didn't or couldn't attend, I strongly recommend holding the dates (Feb 22-25) for the 2012 Medical Quality Meeting at the Buena Vista Palace Hotel in Orlando, Florida. The 2011 meeting's focus was on learning about ACOs and how they can relate to quality of care, prevention and management of chronic illness. Continue reading

Value Based Purchasing

Many of us who work in the quality field have longed for respect. Respect and financial advantage are often tied together. Over the years there have been finally increasing opportunities for pay for performance. Those institutions that have excelled in clinical quality rightly expect to receive some financial benefit for their success. Those payers who have rewarded quality have traditionally chosen to increase reimbursement or providing a bump in payment rather than take money away from those who don't meet the expectations. Other payers have taken the position that quality is expected and does not warrant a payment premium. Continue reading

The QUEST Continues: Achieving More Effective and Efficient Care Through a Collaborative Execution Model

What began in January 2008 as an experiment in collaboration and transparency among acute care providers is now proving to be significant method to accelerate the delivery of high value healthcare. Background Beginning with a group of 169 hospitals in 2008, and growing to include a present day cohort of 220, the Premier QUEST hospitals set about to transform the way value is defined, measured and achieved in healthcare delivery. This was a varied set of initial hospitals whose members ranged from small rural community hospitals to large inner city teaching institutions. However they all shared one common goal: to deliver high value healthcare as defined broadly by five domains. Within each of these domains, reduction of hospital wide mortality, delivery of evidence based care, prevention of harm, improvement of patient experience and reduction of cost, the group defined a measurement process and together agreed on a top performance threshold to which they would each hold one another accountable. All members of the collaborative agreed to three things: 1) that they would be open and transparent with all data and results within the cohort. 2) that they would proactively share the best practices and methods which each found to be successful, and 3) that the institution would have executive sponsorship at the highest level of the organization. Antibiotic Stewardship

In discussion of antibiotic stewardship, four key areas will be examined. First, what are the clinical challenges we face to justify the time and expense of having a program in place. Second, what is the proven value such a program can provide to the patient community and healthcare organization. Third, what is the clinical evidence available in the medical literature to support this effort. Fourth, what is the evidence for various elements of a stewardship program in order to assure success.
