The new AMA Guides to the Evaluation of Permanent Impairment allow a rating based solely on patient "subjective" report of loss of function. The Guides include a sample one page simple questionnaire. We will make a copy of this form for you and explain how to use it in your practice based on Dr. Immerman's 55 trials and 37 depositions in the past 10 years.
