Reporting Workplace Incidents and Dangerous Events
 
Employers, principal contractors and self employed persons are required by the Queensland Workplace Health and Safety Regulation 2008 (Qld), to report 'notifiable' workplace incidents within 24 hours of becoming aware of the incident (or immediately in the case of a death).
 
A notifiable incident is defined as:
  • Serious bodily injury (loss of part or organ from body, an absence from work exceeding 4 days as a result of a work related injury, or a death);
  • Work caused illness;
  • Dangerous event (implosion, explosion, collapse etc);
  • Dangerous electrical event;
  • Serious electrical incident; and
  • Major accident under the DGSM Act.
 
The maximum penalty for failing to notify Workplace Health and Safety Queensland is 20 penalty units which is $2000 for an individual and $10 000 for a corporation.

How to report an incident
Use the online form or download the approved form to notify Queensland Workplace Health and Safety of the incident.
If the incident involves a death, immediate notification is required on 1300 369 915.
