Presenting Health Questionnaire #3 to the Patient

MAKING IT EASY!

 It is not uncommon to encounter some patient resistance when introducing new systems and paperwork in your office. We have found that the time taken to complete the Health Questionnaire is greatly reduced when office staff spends just a few moments educating the patient on certain areas of the Health Questionnaire (HQ3) form. Here are some tips that can save valuable staff time and help eliminate some common errors and warnings.
 
- TIME SAVING TIP #1 - Assist the patient with marking their primary complaints
 
- TIME SAVING TIP #2 - Present the patient with Page 2 of the HQ3 and ask them to complete the upper portion with what aggravates or relieves their symptoms. Let them know that in this section, when an answer inside a shaded pink area is marked, no other answer can be marked.
 
- TIME SAVING TIP #3 - Show the patient the headache section. Let them know that the doctor may be able to help with headaches. If they have experienced headaches recently, have them complete this section. If not, skip and move to the next section.
