LASER TREATMENT OF THE SKIN and its EVOLUTION

dr cassilethThe use of lasers to treat facial problems is a relatively new procedure. Many different lasers have been, and are being developed to treat specific conditions. They can be roughly grouped into two categories depending on their wavelength, i.e. ones that target specific types of cells, like hemoglobin in blood vessels (pulse dye laser) and those that are ablative, that just vaporize everything they are focused  at, like the erbium and carbon dioxide lasers(CO2).

The latter are the type primarily used in facial rejuvenation. The depth and extent of vaporization is controlled by various modalities, like the length of time that the laser is focused on an area, the size of the spot that comes out of the laser machine as well as the laser's power setting and frequency of the pulses emitted. The smaller the spot size, the lower the power output and the longer it is focused on an area as well as how many times it is passed over the same area determines the amount of surface tissue it will remove and damage to the surrounding tissues it will cause.

Essentially, a burn is produced which may be superficial (1st degree) down to a deep or 3rd degree. The depth of the burn will determine how long it will take to heal. A superficial ablation or burn of the skin surface by the laser will produce some redness or inflammation and take a few days to go away. The deeper 2nd degree level will cause some blistering, scabbing as well as redness (inflammation) which should still heal within a two week period. The latter though may persist for several months until it heals. If the depth of penetration of the laser causes a 3rd degree burn, then scarring would result.

Facial resurfacing with the CO2 laser became extremely popular because it was very effective in rejuvenating the skin, helping to reverse primarily the textural changes associated with aging, sun damage, and smoking. This laser almost totally replaced dermabrasion in the treatment of acne scarring. When properly used, the complications were minimal, but used incorrectly could be disastrous creating extensive scarring. The side-effects, such as prolonged redness (average of four months with limited sun-exposure) and difference in coloring from treated and non-treated areas became more troublesome. Both of these usually faded with time but others were left with whiter faces (caused by the removal of any existing tan which is confined to the outermost layers of skin) redundant creepiness and uneven coloration of the neck. Also, because only the face has "thick" skin, treatment was limited to this area.  If these conditions were acceptable, the results of the treatment of the face could be quite dramatic!

In order to minimize these side-effects, a "gentler" laser, the erbium (often referred to as "Fraxel"), was introduced for facial resurfacing. This increased the margin of safety, decreased the side-effects, and length of healing, but also compromised the results. The Erbium does not penetrate as deeply as the CO2 laser. It takes about five passes of the Erbium laser to equal one of the CO2 laser! The Erbium laser treatment, however, no longer required an anesthetic but could be done with a topical ointment.

The next major advance was the introduction of a fractionated laser, first the Fractionated Erbium and then the Fractionated CO2 laser. Essentially a grid is placed on the opening of the laser which blocks a portion of the beam that is emitted by the laser. So, instead of a whole area that is treated, the shutter effect limits the burned area by the laser to a certain percentage (about 50% plus) only to be ablated or burned. This limits the burn which limits the pain and also allows treatment with only a topical anesthetic.

The untreated remaining skin suffers less damage/burn resulting in less inflammation (redness), usually minimal blistering and scabbing and a shorter time of healing.  On average this is between one to two weeks with minimal, if any, redness persisting. Also, there is less of a need to protect oneself from the sun to prevent post-treatment color changes (hyper-pigmentation). Finally, and quite significantly, other areas besides the face, like the neck, chest or even hands can be treated.

Because of the above benefits, the Fractionated CO2 laser has become the instrument of choice for most plastic surgeons and dermatologists in treating sun-damaged, aging and smokers' skin that show textural changes. The Fractionated Erbium laser, just like its predecessor requires many more passes or several treatments to achieve the results of the Fractionated CO2 laser. Contrary to popular belief, neither laser causes more than a temporary skin tightening.

The laser world has evolved and improved in its quest for the most effective treatment with the least amount of side-effects or complications. The guidance of your plastic surgeon or dermatologist is still important to achieve the best possible improvement for your level of skin damage.

DogDid you know that stroking your dog prompts your brain to release endorphins or 'feel good' hormones? Families who own a dog are not as stressed and visit their doctors less often than those who don't, say scientists from Cambridge University.  They help you get your daily exercise and make you feel optimistic and relaxed, which lowers your blood pressure. Dog owners are less likely to suffer anxiety, loneliness and have more confidence than people without dogs. Some animals can warn their owners about oncoming illness and there are many reports about dogs detecting cancer and some can identify other types of illness, such as hypoglycaemia (dangerously-low blood sugar levels) in diabetics and oncoming epileptic fits.  
 
Scientists are just beginning to scratch the surface on the intelligence level of our furry canine friends.  Dogs can understand more than 150 words and intentionally deceive other dogs and people to get treats, according to psychologist and leading canine researcher Stanley Coren, PhD, of the University of British Columbia.

He spoke Saturday on the topic "How Dogs Think" at the American Psychological Association's 117th Annual Convention. Coren, author of more than a half-dozen popular books on dogs and dog behavior, has reviewed numerous studies to conclude that dogs have the ability to solve complex problems and are more like humans and other higher primates than previously thought.

"We all want insight into how our furry companions think, and we want to understand the silly, quirky and apparently irrational behaviors that they demonstrate," Coren said in an interview. "Their stunning flashes of brilliance and creativity are reminders that they may not be Einsteins but are sure closer to humans than we thought." According to several behavioral measures, Coren says dogs' mental abilities are close to a human child age 2 to 2.5 years.

The intelligence of various types of dogs does differ and the dog's breed determines some of these differences, Coren says. "There are three types of dog intelligence: instinctive (what the dog is bred to do), adaptive (how well the dog learns from its environment to solve problems) and working and obedience (the equivalent of 'school learning')." Data from 208 dog obedience judges from the United States and Canada showed the differences in working and obedience intelligence of dog breeds, according to Coren. "Border Collies are number one; poodles are second, followed by German shepherds. Fourth on the list is golden retrievers; fifth, dobermans; sixth, Shetland sheepdogs; and finally, Labrador retrievers," said Coren.

As for language, the average dog can learn 165 words, including signals, and the "super dogs" (those in the top 20 percent of dog intelligence) can learn 250 words, Coren says. "The upper limit of dogs' ability to learn language is partly based on a study of a border collie named Rico who showed knowledge of 200 spoken words and demonstrated 'fast-track learning,' which scientists believed to be found only in humans and language learning apes," Coren said.  Dogs can also count up to four or five, said Coren. And they have a basic understanding of arithmetic and will notice errors in simple computations, such as 1+1=1 or 1+1=3.

Four studies he examined looked how dogs solve spatial problems by modeling human or other dogs' behavior using a barrier type problem. Through observation, Coren said, dogs can learn the location of valued items (treats), better routes in the environment (the fastest way to a favorite chair), how to operate mechanisms (such as latches and simple machines) and the meaning of words and symbolic concepts (sometimes by simply listening to people speak and watching their actions).

During play, dogs are capable of deliberately trying to deceive other dogs and people in order to get rewards, said Coren. "And they are nearly as successful in deceiving humans as humans are in deceiving dogs."

If you are thinking about adding a furry canine friend to your family, here is a quick "Puppy Test" to help you make the right choice:

1) Place the puppy in a separate room. Remove him/her from the chaos of the kennel and the other pups, and then move away. Does he hide in a corner, bolt directly to you, or investigate his new surrounding? You want a dong that's interested in your first, then his environment.
2) Crouch down and clap your hands.  The dog should come to you, tail up (which indicates friendliness) - but not bite or wrestle with your hands which is a sign of dominance.
3) Roll the puppy gently on his/her back.  If the puppy resists or bites, that's also a sign of dominance.
4) Do the touch test.   Slowly stroke the length of the puppy's back 3 times. Does he/she lean in toward you (good), or subtly pull away (not as good)?  Pick the puppy up in one hand. Cuddliness is good, fighting, struggling, and biting, not so much.
5) Go on a Speed Date.  Spend 20 full seconds patting the dog and talking to him/her. If he/she doesn't stick around, the puppy is likely to be more independent and harder to train.
6) Try Paper Play.   Toss a crumpled paper on the floor in front of the dog.  Does he/she pounce on it and "fetch" it? That's a dog that's going to want to work with you.

Botox Question:

1) How effective are the home-use photo-rejuvenation devices (like Omnilux New-U, DPL System, and LightStim) for wrinkles and hyperpigmentation?  Are they worth the money?

2)  I had the Restylane filler in the past. However, due to the economy today, I am unable to use the Restylane filler anymore, however, I would like to have something comparable but at a lower cost. Is there anything available or are there any dermatology schools, where the cost would be less?  My family reunion will be held in the fall and I would like to look my best.  I am 60 years old and do find that the filler has lifted my face.  It seems I am a drooper and my face becomes squared off.  I am hoping that you can guide me in the right direction so that I can somehow maintain this procedure during these challenging economic times.

1)  This one is proving devilishly difficult because it's been hard to get out of these companies what they are actually using in terms of power and wavelengths.  Even the "physician" company websites contain just what I would term marketing materials and very little substance.  We're still working on this, calling the companies etc. and trying to get studies.  Most of the studies I've seen so far were very small, not controlled and done by docs being paid by the company to do them (limited usefulness).  I'll keep you posted as we get more info.  My general sense of the "wrinkle" home use devices, from conferences and reading, is that they don't do much.  These as opposed to the Tria, a home use hair removal device, which does work well.

2)  I really sympathize with your desire to maintain your skin without quite so much cost.   Sometimes the Restylane/Juvederm done in salons is a little less but often that's because there is a "medical technician" (someone with limited medical training) doing the procedure.  You might be able to find a salon or spa who has an experienced RN injector.  Look for an RN with at least 5 years of experience.  The price could be lower.
Or, you might try establishing yourself with an office that is larger and does a lot of Restylane/Juvederm.   Sometimes they will have training days (we do this at our office) where the company will supply the filler and we will work/teach with patients at no charge.  The purpose is to advance the skills of all the younger doctors, PAs, RNs who are providing fillers in the office.  You could volunteer to be one of those patients.  But, you have to do it on the office's training time schedule and that may not work for you if you have an event.

Fillers or Photoshop?

EveMichaelsI read an interesting article about Photoshop in the Los Angeles Times last week. It showed a very pretty woman's before and after pictures. In the "before" picture, the woman had deep crow's feet that were distracting from her beauty, to say the least. In the "after" picture, the smile lines had mostly been erased, allowing one to focus on the overall beauty of her eyes and face.

I started thinking that if she had a bit of Botox under her eyes, she wouldn't have needed much retouching, if any at all. Then I realized that I personally prefer to get Botox and fillers done on a regular basis to keep my face looking camera ready at all times.

So, you're probably thinking, that's what Photoshop is for. Well, yes and no. According to the debate in the Los Angeles Times article, it was intended to erase shadows, smooth obvious flaws, brighten skin tone, etc. It was never intended to totally "remake" the person so that they looked unrecognizable in person.

I'm certain that many of you remember seeing the movie Clueless years ago starring Alicia Silverstone. Alicia always took a picture of herself in her outfit, before wearing it, so that she could see what she really looked like. She aptly said, "The camera never lies!" Truer words have never been spoken. The camera may distort, but lie? Never.

At SimplyAgeless411, clients continually ask us if they should have Botox or fillers. When we advise them which injectables they need for specific areas and they often have a difficult time acknowledging what has aged on their faces. In other words, they "know", but they don't really want to "know". It's kind of like how everyone seems to look great in his or her own bedroom mirror. A few years ago this wasn't even an option. It was under the knife or nothing. Now we can rejuvenate our faces in real life...often times in just one to two office visits.
