The "Other" Lip Enhancement

OtherMany women have vaginal problems or defects as a result of genetics or developmental issues and others as a result of pregnancy. Therefore it is not unreasonable to consider labiaplasty surgery or vaginoplasty surgery even before having children, in fact, most patients have never been pregnant before their corrective labiaplasty surgery.

Labiaplasty and Vaginoplasty are among the fastest growing and accepted cosmetic procedures for women. It involves the surgical modification (most frequently reduction) of either the labia minora (inner lips) or labia majora (outer lips) to achieve a more proportional and/or comfortable anatomic size. The procedure is done on an outpatient basis and typically requires only light sedation or twilight sleep anesthesia. Recovery is relatively quick with most everyone back on their feet the next day and resuming most activities in about a week.  Sexual intercourse may commence after 4 weeks following Labiaplasty and 6 weeks following Vaginoplasty. Even relative to many other plastic and cosmetic surgeries, labiaplasty and vaginoplasty patients have a high satisfaction rate and an improved self-esteem after surgery.

Labiaplasty is in a category of vaginal plastic surgeries known as female genital cosmetic surgery (FGCS). FGCS also includes:
  1. Hoodectomy whereby excessive tissue covering the clitoris is reduced to provide better clitoral stimulation and thus improved arousal.
  2. Vaginal Rejuvenation in which the vaginal vault is tightened and supported to provide better accommodation and thus heightened sensation during sex.
If you are a good candidate and if necessary, labiaplasty, vaginoplasty, hoodectomy, vaginal rejuvenation and/or vaginal reconstruction can often be performed at the same time for convenience and to avoid multiple recovery periods.

What makes a good candidate for labiaplasty and/or vaginoplasty?

Aesthetic Concerns:
     - Excessive length, thickness, bulk, redundancy, and/or dark pigmentation

Artificial Knees Made to Order

A startup company is taking a customized approach to knee replacement surgery, creating knee implants on demand that exactly match a patient's anatomy. The company, ConforMIS, based in Burlington, MA, is bringing the technology of rapid prototyping, which converts a three-dimensional computer design into a physical object, into the field of orthopedics. The company believes that such custom-made implants can make knee replacement surgery faster, more accurate, and less traumatic to the patient.

Knee replacement surgery repairs damage and relieves pain in patients with severe osteoarthritis or knee injury. Total knee replacement involves removing diseased cartilage and bone from the surfaces of the knee joint--the thigh bone, shin bone, and kneecap--and replacing them with an artificial joint made from a combination of metal and plastic. A partial knee replacement can also be performed on one part of the joint.

Typically, a surgeon chooses an artificial joint from several options of different sizes. ConforMIS, however, creates a custom implant based on imaging data of a patient's knee joint, a technology that the company calls iFit. ConforMIS transforms images from CT or MRI scans into a three-dimensional computer model using computer-aided design (CAD) software, which serves as a template for manufacturing the implant.

Philipp Lang, president and CEO of ConforMIS, says that typically, surgeons must shape the patient's bone to fit the implant. Because the bone-facing side of ConforMIS's implant is matched to the patient's anatomy, it can reduce the amount of bone that the surgeon must cut. On the joint-facing side of the implant, Lang says, "we want to re-create the geometry that the patient had before he or she developed arthritis." He says that the ability to fit the implant to the patient without removing excess bone will speed recovery time and lessen pain.

Along with the implants, ConforMIS also creates customized instruments, called iJig, which assist surgeons in placing the implant. Because the knee joint must be precisely aligned to function properly, surgeons use cutting and placement guides to ensure that any cuts to the bone are made at the correct angles so that the implant is placed in proper alignment. ConforMIS uses the imaging data from the patient to create disposable instruments that are calibrated to the patient's specific implant and anatomy. Lang says that the instrumentation will enable surgeons to perform the operation more quickly and easily, and it will also reduce time spent sterilizing reusable equipment.
 
Currently, the company has released three products for partial knee replacements, along with instrumentation, and it plans to make an implant available for a total knee replacement in 2010. Lang says that it takes about four weeks to create the implant, from imaging to shipping. The company hopes to provide its customized products at the same price as that of a standard implant.

Ask the Doctor Question
 
Some doctors recommend fat transfers as facial fillers to fill out wrinkles and improve hollowness in the face while many doctors recommend using facial fillers such as Juvederm, Sculptra, Radiesse or Restylane for the same problem.  Can you tell me the main difference between these two treatments and which method works the best?
 

Answer:
 
This is a very common question as it can be a bit confusing.  Essentially, as one ages we lose volume or the fullness in our face.  It turns out that this is as important if not more important than the excess skin we develop.  Hence, the treatment for this component of aging is simple but yet complicated.  We need to fill in the areas that have lost volume, such as the cheeks, but with what?  Now to the heart of your question...once you have determined that a filler is needed to treat the face, next you must determine what kind. 
 
Today there a numerous options and each has its own pro's and con's.  Simply stated, fillers can be divided into permanent and temporary fillers.  Most patients today tend to use temporary fillers such as Restylane or Juvederm.  They are safe, well studied, most have limited side effects and good results when injected by a skilled practitioner.  Most importantly, they are temporary - hence, if there is an error or one is unhappy with the appearance, it will eventually dissolve.  This same advantage, is the main disadvantage with temporary fillers, in that once a patient is pleased with the affects of a temporary filler, they often wish it would last forever so they wouldn't have to keep repeating the injections. 
 
Okay, this leads me to fat.  Fat is considered a permanent filler, that is, whatever of the fat survives the transfer.  Fat is harvested from one's own body and this harvesting technique is crucial to the success of the transfer.  It is carefully transferred to areas that need volume replacement.  Sounds great-however, there are many things to consider with fat that need to be understood prior to jumping on the table.  First of all, fat transfer is usually not a one-time procedure but rather a 2-3 time procedure because all of the fat that one injects does not survive the transfer and often the initial great results fade with time.   Hence, repeated rounds are necessary. 
 
Secondly, fat transfer is much more unpredictable than a temporary filler such as Juvederm.  One can never know how much fat will survive and on which side of the face....so be careful.  Furthermore, if a mistake is made, poor judgment is used or you just don't like the way it looks, removing it is incredibly difficult if not impossible.  Finally, the downtime is much greater as this is like a mini-surgery, thus, more expensive in the short run, although in long-run it will save you much more money. 
 
In conclusion, for the novice filler patient or one that does not want to hassle with long recoveries and potential risks, stick to temporary fillers.  For patients that are more tolerant of risks and wish for a longer and often more natural result, then fat grafting is an excellent option.  The only advice is that it should be done by someone with experience.....

Few, M.D. an Assistant Professor of Plastic Surgery at Northwestern University's Feinberg School of Medicine located in Chicago believes this trend is sure to continue.
 
Young people in their 20's and 30's are opting for Botox injections to slow or stop wrinkles, choosing to get these injections at the first sign of wrinkles. Botox stops the muscles from contracting by blocking the transmissions of signals from the nerves to muscles.
 
Some cosmetic surgeons believe that by starting this early will result in less work being needed later on. Dr. Few believes that using Botox or other similar anti-aging treatment to control wrinkles early on may prevent the development of deeper creases as the person ages.
 
In addition to Botox and other dermal fillers, the younger generation is also big on microdermasion and laser skin resurfacing to remove the outer signs of aging caused from environmental factors. It is a well known fact that sun exposure causes the skin to age, so by resurfacing, it is hoped that this effect is minimized.
 
The American Society for Aesthetic Plastic Surgery (ASAPS) statistics for 2008 had 102,068 persons aged 19-34 opting for laser skin resurfacing and 9,156 of those 18 and younger also getting this procedure done.
 
Statistics from the American Society for Aesthetic Plastic Surgery for 2008, shows that the number of Botox injections for persons aged 19 to 34 was 505,363 and 7,915 for persons aged 18 and younger. For other soft tissue fillers such as Restylane the numbers were 260,988 for the 19 to 34 age group and 5,423 for those aged 18 and under.
 
Another top anti-aging procedure for the 19 - 34 age group was microdermabrasion with 249,877.    
 
The vice president of The American Society for Aesthetic Plastic Surgery (ASAPS) believes in starting patients young, guiding them towards proper skin maintenance as a first step. Others in the industry also believe that proper protection from the sun and caring for the skin is the best way to help minimize the amount of work that may be needed as persons reach their 50s and 60s.
