Lessons Learned: It's about change
Projects by nature are unique. As many as I have worked on over the years, each has left me with a lessons learned list that I attempt to apply to the next project. However, each project in its own unique way, provides a new set of challenges and lessons to be learned for the future.
If your project has multiple phases, I encourage you to host a post-phase briefing with your team to discuss lessons learned and ways to improve the subsequent phases. Don't be surprised by what you hear, each team member has a unique perspective which is a valuable contribution to future phase success.
Document the lessons learned and remind team members when old habits re-surface that the same result is inevitable unless we learn to change our ways.
To Quote Albert Einstein: "We can't solve problems by using the same kind of thinking we used when we created them."

It is often said that a picture is worth a thousand words. Diagrams, flow charts, etc are invaluable when trying to unpack these 'complex processes'. Sometimes, you may find that these 'hideously complex' processes may be simplified when looked at as part of the larger 'picture'.
