Welcome to the second newsletter of the Shared Decision Making (SDM) program. As part of a care team at MGH, you will receive a quarterly newsletter that includes: updates to the available video programs and shared decision making projects, practice-specific prescription data, upcoming events for patients and providers, as well as useful resources and links.
The Shared Decision Making program is sponsored by the MGH Health Decision Sciences Center and John D. Stoeckle Center. We strive to provide high-quality health information to promote active and shared decision-making in clinical care at the Massachusetts General Hospital.

Pre-Visit Prep for Annual
Wellness Visits
Karen Sepucha and Leigh Simmons participated in the Partners Clinical Process Improvement Leadership Program (CPIP) this spring. Their project for this CPIP course focused on providing decision aids in advance of wellness visits for patients in the Internal Medicine Associates (IMA). They worked closely with one pod in the IMA over the course of 4 months to implement a process by which patients could order decision aids in advance of their annual physicals, using an order form mailed with their appointment reminder letters.
Drs. Blair Fosburgh and Alaka Ray also participated in a pre-visit preparation process in which they ordered laboratory tests and decision aids for patients in advance of their annual visits. Medical assistant Amelia Yorke and patient service coordinator Lisa Lee-Jones were integral to the development of the new process and implementation of the pre-visit workflow. Both physicians remarked that the process took some getting used to, but ultimately led to a better-prepared annual physical and was an efficient use of their time. The team felt that the early preparation gave them a more "high-touch" experience in patient care and was a good exercise in improving team-based care.
These interventions in pre-visit preparation led to 40% of patients receiving video decision aids in advance of their wellness visits. An interesting finding from the patient self-ordering project was that patients tended to order more programs focused on chronic and symptomatic conditions (e.g. Depression, Insomnia, and Menopause) where the physicians tended to order more programs on cancer screening and advance directives. This suggests that the ideal deployment of decision aids in our primary care practices will likely need the input of both patients and their physicians to get these programs to our patients at the time that is right for them.

Shared Decision Making in
Specialty Care
The Shared Decision Making team has been working to expand our reach to specialty care. The team worked with clinicians in OB/GYN as part of their departments' quality improvement program. As part of their QI measure, the clinicians completed a formal decision making needs assessment and watched one of our decision aids. OB/GYN is planning to continue with another QI measure this quarter that will be tied to ordering the decision aids. This project joins collaborations with other specialty groups including: Orthopedics, Breast Cancer, and Cardiology.

Our colleague, Dominick Frosch, PhD, from Palo Alto Medical Foundation, published a new study titled: "Fear of Being Labeled 'Difficult' May Keep Patients from Participating in Shared Decision Making". Click here to read the interesting findings.
Dr. Michael Barry and Dr. Mary McNaughton-Collins provided their opinions regarding the recent USPSTF's final recommendation against routine PSA testing. Shared decision making continues to be an important key factor in this medical decision. |

We're very close to reaching the 10,000th video prescribed since the program began in 2005 -- a milestone! An average of 340 videos were prescribed each month this past quarter. |

Upcoming Events The Blum Center at MGH hosts monthly patient education sessions including the presentation of a shared decision making video followed by a facilitated discussion. The one-hour lunch session, 12:00-1:00pm, is open to MGH patients and staff.
