President's Message
Colleagues,
 
I am writing to provide an update regarding the status of legislation from Rep. Gonzalez for the 2010 CMS Final Rule.
 
In 2011 cardiology practices will face significant cuts for reimbursement for imaging procedures. Rep Gonzalez has introduced legislation that will phase in these cuts over several years, allowing practices to make adjustments to these cuts. Our ACC legislative affairs team has been working with Rep. Gonzalez's staff on legislation. The goal is to have the legislative language finalized by Friday, October 8th and to have the legislation introduced during the Lame Duck session which will be held after the November 2 elections. Our hope is to have this legislation rolled in to the SGR vehicle that must pass before the cuts go in to effect on Dec. 1.  
 
Congress is adjourning on Friday, so they will be back in their districts campaigning up until the elections. Since Congress will be back home, the ACC advocacy staff will be setting up meetings for the last two weeks of October with members of Congress who have signed on to H.R. 4371 - the previous Gonzalez bill - as well as Senators who have expressed an interest in the legislation. They will be calling upon all of you to attend these very important meetings in the hopes of securing as many original cosponsors as possible. In addition there will be email alerts and other calls to action to all ACC members. Please stay tuned.

Results of ACC Practice Census - Indiana Data
Practice CensusA new American College of Cardiology (ACC) survey of more than 2,400 practices across the country provides a comprehensive snapshot of the current state of cardiology given the changing health care landscape. Respondents from 49 states and Puerto Rico provided valuable insight into the various ways the changes are forcing many private practices to take drastic actions to remain viable. Over the last year, the survey found that more than half of all practices have taken some form of cost-cutting action as a direct result of the cuts in reimbursement for cardiovascular services included in the 2010 Medicare Physician Fee Schedule. The first and largest wave of activity is directed at the staff level, with half (50 percent) of cardiovascular group practices reporting a reduction in staff to save expenses. In addition, 40 percent of survey respondents said they have reduced staff benefits, while 45 percent have reduced salaries for physicians and clinical staff (28 percent). A small percentage (3 percent) of survey respondents has chosen to retire or close the practice all together. While some survey respondents indicated an increase in non-physician clinical support staff (10 percent), the total number of new staff fails to compensate for the more than 2,600 nurses, nurse practitioners, CV techs and pharmacists that practices reported needing to lay off.

Indiana-ACC Members Attended ACC Legislative Conference

I want to thank the Indiana American College of Cardiology (ACC) for the fabulous opportunity to attend the 2010 Legislative Conference in Washington, D.C. September, 12-14. There were over 300 physicians, fellows, nurses and healthcare administrators in attendance from across our nation.  This three-day conference provided the opportunity to hear directly from our ACC leaders and legislators as to the issues and concerns with the Affordable Care Act (ACA).   The Legislative Conference also offered additional education regarding the need for repeal/redesign of the current Sustainable Growth Rate (SGR ) used for CMS payment calculations to providers.  Dr. Donald Berwick, Administrator for CMS, offered very inspirational words of his vision and direction for the future of healthcare with a focus on safety, quality and prevention.  The last day of the conference was reserved for Capitol Hill, to meet with our Indiana congressional leaders to ask for their needed support with the Gonzalez legislation, securing  a transition period for proposed cuts in reimbursement.

New ICD-9Codes
Beginning in October, physicians will have 122 new ICD-9 diagnosis codes and 12 new procedure codes from which to choose for 2011. A large portion of the diagnosis codes are in the "V" code section, which describe a "supplementary classification of factors influencing health status and contact with health services," according to the ICD-9 manual. There are a few codes that will be added to the cardiology portion of ICD-9-CM. The lists, from CMS, also include codes that were revised and codes that will be invalid for 2011. New Cardiology Codes are:

  • 447.70 - Aortic ectasia, unspecified site
  • 447.71 - Thoracic aortic ectasia
  • 447.72 - Abdominal aortic ectasia
  • 447.73 - Thoracoabdominal aortic ectasia
