David Dexter, CEO Sonora Quest Laboratories, LLC. David
A. Dexter is the Managing Director for the Quest Diagnostics Operations
in Arizona, serving as President & CEO for Sonora Quest
Laboratories, LLC a joint venture between Quest Diagnostics (a Fortune
400 company) and Banner Health (the largest non-secular health delivery
system in the U.S.) and as President & CEO for Laboratory Sciences of Arizona, LLC which manages the nine hospital laboratories for Banner Health in Arizona. Sonora Quest
Laboratories/Laboratory Sciences of Arizona was recently recognized by
the Phoenix Business Journal as one of 'The Top 25 Best Places to Work
in the Valley' for 2008. Most notably, Sonora Quest
Laboratories has received 5 State Quality Awards from the Arizona
Quality Alliance, including the prestigious 2005 'Arizona Governor's
Quality Award'. David is a Certified Six Sigma Green Belt and he serves on the Board of Directors and Executive Board for Arizona Health-e-Connectivity.
Thomas Pyzdek, Author and Consultant. Tom's
career in business process improvement spans more than 40 years. He is
a leading quality and Six Sigma authority and the author of over 50
copyrighted works including The Six Sigma Handbook, The Handbook for Quality Management, and The Quality Engineering Handbook.
Pyzdek was named "Outstanding writer and author" by The International
Who's Who in Quality. He provides consulting to major clients
in a broad spectrum of industries and has taught Six Sigma and other
business process improvement methodologies to thousands. Pyzdek is a
Fellow of ASQ and recipient of the ASQ Edward's Medal and the Simon
Collier Quality Award for outstanding contributions to the field
of quality management, and the ASQ E.L. Grant Medal for outstanding
contributions to Quality Education. Pyzdek serves on numerous editorial
boards, including The Quality Management Journal, Quality Engineering
and the International Journal of Six Sigma and Competitive Advantage.
He is a past Baldrige Award Examiner.
Andreas A. Theodorou M.D., FCCM, FAAP, Chief Medical Officer, UMC. Dr. Theodorou graduated
from the Wayne State University School of Medicine in Detroit, Michigan
in 1983 and came to Tucson in 1992 after completing a fellowship in
Pediatric Critical Care Medicine. He is a Professor of Clinical
Pediatrics at the University of Arizona and is the Associate Head of
the Department of Pediatrics, Chief of Pediatric Critical Care and the
Chief Medical Officer for the University Medical Center. Dr.
Theodorou has published on a wide variety of critical care topics
including traumatic brain injury, pulmonary hypertension and toxicology
but has most recently focused on error reduction and patient safety.
He has developed a curriculum in Interprofessional Education involving
all four schools at the Arizona Health Sciences Center with an emphasis
on patient safety and error reduction. During his UMC chief of
staff tenure he was involved with the development of the "UMC Quality
and Safety 1st" program and has continued to lead UMC to numerous
quality awards as Director of the UMC Center for Quality and Safety.
Brad Tritle, CIPP. Brad
is a Certified Information Privacy Professional (CIPP) with a B.A. with
Honors in Asian Languages, and an Asian Studies Certificate, from
Arizona State University. He is a member of HIMSS, HL7, the HIMSS PHR
Clinical Task Force, and the American Telemedicine Association. In
September 2007, due to his growing expertise and activity in the area
of e-health, Mr. Tritle was chosen by the Arizona Health-e Connection
(AzHeC) Board to become AzHeC's first full-time executive director. In
this capacity, he leads statewide efforts to ensure coordination of
health information exchange efforts, and promote adoption of health
information technology through education, policy development, and
leading or supporting specific initiatives and projects.
Dr. Neil West. Dr.
West is coeditor of the quarterly FOCUS newsletter for the American
College of Medical Quality (ACMQ) and also serves on the curriculum
committee for the Annual ACMQ meeting each year. He is a board
certified pediatrician, lifetime member of American College of
Physician Executives (ACPE), graduate of Baylor College of Medicine.
For the past two decades, Dr. West has been actively engaged in
measuring processes of medical care in health systems both on the
provider side and insurance entities. One key to changing behavior is
clear data information to strategic stakeholders. He has developed and
implemented Pay for performance (P4P) covering 500,000 + patients in
some 1,000 practices across the U. S..
EMR and PI Speakers
Kenneth Adler, MD,MMM, Medical Director of Information Technology for Arizona Community Physicians (ACP). Ken
led the selection and implementation of ACP's EHR and continues to
provide the main physician leadership for the maintenance and upgrades
to the system. He has published a number of articles on EHRs and
related topics in peer reviewed journals. Dr. Adler is a juror for the
Certification Commission for Health Information Technology and an
independent health IT consultant. Dr. Adler has completed the
HIMSS/University of Connecticut program in Health Information
Technology and the MIA/OHSU 10/10 program in Biomedical Informatics.
He is a Certified Professional in Healthcare Information and Management
Systems and a Fellow of the Healthcare Information and Management
Systems Society.
Anita Bach, Director of Cardiac Services for Tucson Medical Center.
Eric Bergstrom, Pharm.D. Pharmacist at Tucson Medical Center. Eric
Bergstrom is the Clinical Lead Pharmacist at Tucson Medical Center(TMC)
and has been there for over 16 years. He earned his Doctorate in
Pharmacy at the University of Arizona in 1993. He was the first to
complete a General Pharmacy Residency at Tucson Medical Center in
1994. He has experience as a Master Change Agent. Dr. Bergstrom is a
faculty/preceptor for many pharmacy colleges including the University
of Arizona, Midwestern University Glendale, and University of Southern
Nevada. He has received awards for Preceptor of the Year through the
University of Arizona. He is actively involved in the implementation
of electronic medical records (EMR) at TMC.
Brian Bersbach, Pharm.D. Pharmacist. Brian
has worked for two years as a pharmacist. He was a hospital pharmacy
tech/intern for 5 years prior. Participated in and was an officer of
student pharmacy organizations (ASP, IPSF).
Dr. Kalyanraman Bharathan. Dr.
Bharathan is an economist and a systems engineer who has been involved
with process management and improvement in health care since the
mid-1990s. As a member of the faculty in the Department of Medicine,
he has worked on clinical processes in the outpatient setting,
including studying the role of electronic communications in referral
management and patient care. He has been part of the senior management
at University Physicians since 1998, where he has directed the Quality
Improvement and the Revenue Interface Management Departments, run a
medical records office, and worked on a range of process improvement
activities. He has been the Project Director of the Southern Arizona
Health Information Exchange since 2006. Dr Bharathan is a Certified
Professional in Health Care Quality, and has been President of the
Arizona Association of Health Care Quality (2006), and a member of the
Leadership Council of the National Association of Health Care
Quality.
Peter Bersbach, Master Black Belt. Peter
is a results-oriented Master Black Belt with significant experience
(over 34 years) managing and executing process improvement projects.
His major strengths include Statistical Techniques, Lean Application,
Critical Chain Projects, Teaching and Facilitation Skills, Listening,
and Working with Diverse Teams.
Leslie Bromberg, Branding/Marketing Consultant.
Leslie
is a seasoned professional with over 20 years of experience in
executive management. She has been developing, building, and
implementing brands in the technology and health care sectors for over
15 years. In 1997 she founded Bromberg Consulting, Inc., a consulting
firm specializing in branding and strategic marketing for a diverse
client base and has launched dozens of companies, products and services
nationwide. In 2003, Kaiser Permanente selected Bromberg Consulting to
brand and launch their organization-wide electronic medical record
(EMR) system. In addition, Leslie led a multi-year initiative to gain
insight into deployment lessons learned and the resulting best
practices.
Larry Dyer, Lean Six Sigma Master Black Belt and Director of Process Improvement for Coventry Healthcare. Larry
has over 30 years experience in Information Technology, ranging from
programming to a member of senior management. His primary focus has
been in three areas: (1) Quality Assurance/Process Improvement/Six
Sigma, (2) Strategic & Tactical Business Planning, and (3)
Organizational Development. Mr. Dyer has 13 years of software process
improvement experience and has functioned as a Lean Six Sigma black
belt for the past five years performing organizational process
improvement. In 2008, Larry studied and obtained his Master Black Belt
certification and is now the Director of Process Improvement for
Coventry Healthcare in Tucson. He has used Lean Kaizens extensively to
quickly identify and solve process problems.
Sharby Kennard, Manager ~ Service Excellence, Chandler Regional Medical Center. With
over 10 years experience in Customer Service, Sharby fell into the
specialty of Customer Satisfaction and has worked in industries ranging
from retail, software, financial and most recently in healthcare.
Sharby started her journey specifically focused on customer
satisfaction and customer driven innovation at Intuit Inc for the
QuickBooks product line. It was here where she also began her journey
into process excellence and Six Sigma. After successfully leading a
Global Customer Satisfaction Program for American Express Sharby most
recently made the journey into healthcare where she currently leads the
Service Excellence Program for Chandler Regional Medical Center, part
of the Catholic Healthcare West (CHW) family which is one of the
largest healthcare systems in the West with 42 hospitals in Arizona,
California and Nevada.
John Kemmer-MBA, BSME. John has over twenty
five years experience in various aspects of manufacturing for the
telecom, automotive, materials, aerospace and medical device
industries. His background includes process engineering, quality
management, manufacturing management and continuous improvement. He
considers himself a life-long student of the Toyota Production System.
He has also been certified as a Six Sigma Black Belt by the American
Society for Quality and is a member of the International Society of Six
Sigma Professionals. John currently manages the Lean Systems
Office at Roche Tissue Diagnostics (RTD) in Tucson, AZ. The RTD
manufacturing facility has recently been recognized as "Lean Champion"
by the Association for Manufacturing Excellence.
Dr. Chris Heller. Dr. Heller is a graduate of Johns Hopkins School of Medicine And did a
general Surgical Residency At University Hospitals of Cleveland. He
performed general and vascular surgery in Tucson for 20 years and in
1987 was a co-founder of the Midas+ software company. He has been a
Quality consultant for 20 years and has a long standing interest in
measuring quality performance.
Kathleen Tanner, Master Black Belt and Sr. Performance Improvement Analyst, Tucson Medical Center. Kathleen
Tanner, BA, MA, has worked in the healthcare quality field, for more
than 18 years. She has held positions with several Tucson hospitals
including Quality Coach, Coordinator for Total Quality Management,
Director for Quality and LSS Black Belt & Master Black Belt. She
received her quality training, from a variety of organizations
including: 3M, Statistical Process Controls, Inc. (Don Wheeler, PhD)
and General Electric Healthcare Performance Solutions.
