School age children must have a full evaluation every three years (a yearly update or annual review is required) if therapy is school related; outside of school, annual evaluations are required. Section II, 214.300 C-13 - for PT & OT, Section II, 214.400 D-11 for speech

"School related" means the child is of school age, attends public school and receives therapy provided by the school.

Pages one (1) and two (2), the Goals and Objectives page (pertinent to the therapy requested) and the signature page of the Individual Education Plan (IEP) are required for verification as review documentation. Section II, 204.000 A/4-5 
Reminders!

The qualified therapist or speech-language pathologist must review and approve all written documentation completed by the individual under his or her supervision prior to the filing of claims for the service provided. Each page of progress note entries must be signed by the supervising therapist with his or her full signature, credentials and date of review. 203.000 C-1.
