Reminders
1. Objective information in evaluations should be stated in measurable terms.
For example: Decreased range of motion should be stated with objective criteria such as goniometric measurements or functional limitations. Increased muscle tone should be stated with objective data to support the statement such as scores on the modified Ashworth scale.
Providers who state subjective findings such as "increased" or "decreased" should provide objective data to support finding. This is most common in statements involving range of motion, pain, tone and strength.
Objective findings must be provided as goniometric measurements, pain scale, manual muscle test or the Modified Ashworth Scale.
2. When calculating a baby's adjusted age, it is figured from 40 weeks gestation. For example: a child born at 24 weeks has an adjusted age of 16 weeks younger than their chronological age. Infants are not considered premature unless they are born at 37 weeks or below.
3. Do not send overnight deliveries for delivery on Saturday or Sunday. The QSource of Arkansas office is closed on Saturday and Sunday. No deliveries can be received. Please don't waste your money.
4. If a child qualifies for therapy based on standard deviations/test scores, clinical observations/impressions still need to be included, even if the child meets the objective criteria for therapy.
5. School-age children must have a full evaluation every three years (a yearly update is required) if therapy is school related. Outside of school, annual evaluations are required. "School related" means the child is of school age, attends public school and receives therapy provided by the school.
6. IQ scores are required for all children who are school age and receiving language therapy. Exception: IQ scores are not required for children under 10 years of age.
7. Each quarter providers receive their "purple" sheets (chart requests) for that quarter's sample. The sheet tells you the "chart request date" - usually the 10th of the month. Providers must have their selected records in within 30 calendar days from the request date. Make sure you count the days. For example: Jan. 10 was the most recent request date, which means records were due Feb. 9.
