It's important to calculate corrected age for kids accurately. This is required to be included in therapy evaluations for all kids 12 months of age or younger.

Here's the formula to do it, compliments of the American Academy of Pediatrics:

Subtract the number of weeks born before 40 weeks of gestation from the chronological age.

Therefore, a 7 month old, former 28 week gestational age infant, has a corrected age of 4 months according to the following equation:

7 months - [(40 weeks - 28 weeks) / 4 weeks ]
7 months - [(12) / 4 weeks ]
7 months - [3]

Private providers giving summer therapy to school age kids (who receive therapy in school during the school year) are required to do an ANNUAL evaluation.

Any corrections made to a DMS-640 form (ie. treatment box checked, minutes or duration of therapy added, diagnosis change or addition, date physician signed) must be made with medical integrity and denoted on the form as a correction.
Schools: Eliminating Provider Type 42

Arkansas Medicaid will be eliminating public school use of Provider type 42 for therapy services.

If you are a public school and you bill under an AR Medicaid provider number ending in "42", you will not be allowed to use that number starting Nov. 1, 2010.

All public schools will have to use a provider number ending in "43"-which designates them as a School. If you are a public school and do not have a provider number ending in "43", you need to get one in order to bill for school based therapy services.
 
Goals & Objectives for AR Medicaid Children

Goals and Objectives for Arkansas Medicaid Children receiving therapy services must be written measureable, functional and individualized.
