Dear patients, friends and colleagues,  

 

Welcome back from the holiday season! I hope that everyone enjoyed time with family and friends. Now the hard part is here, getting ourselves through the next few months of cold weather. It is a great time to get a lot of work done!

 

In this issue, we are going to talk to you about Xeomin, a new botulinum toxin. I will explain how we think this might be helpful as an addition to our other toxins, Botox® and Dysport™. It will mostly be helpful for those of you who have found that either of these have not worked as well as they had in past treatments. 

 

We are going to be deploying the final stage of our new electronic medical records, Nextech, in early February. We are excited about the fact that you will be able to do most of your paperwork online before your visit AND you will never have to write your name twice again! We know that many of you loathe the paperwork at our office, but seriously, we need to have accurate information and this changes frequently. So, we will still be asking you to update your information, but you will not ever have to enter your primary information again if it hasn't changed. You will be able to input information about the visit that is coming up before you get here, making your visit go more smoothly and efficiently. Please feel free to make comments and suggestions during this period, or at any time for that matter. We always try to make changes, make your visits better and your experience at Advanced Dermatology and skinfo® top notch.

 

We are also rolling out some new services (as usual), which include new light treatments for acne, new esthetic services and we are also installing a more powerful and faster Xtrac, the laser device for psoriasis that will allow us to treat up to 20% body surface area! This means that in the past if you thought that you had too much psoriasis to get the laser but you were still uncomfortable going on an injectable medication, now is the time to come forward. This treatment is covered by almost all major insurance carriers and is very effective and safe.

 

I have been to a number of meetings recently including the American Society of Dermatologic Surgery in Washington DC and the American Society of Cosmetic Dermatology and Surgery in Las Vegas where I was honored to be on the faculty at both. I learned many pearls of wisdom from my colleagues around the country who I have come to know through these meetings. It is a wonderful way to share knowledge and get up to speed on the best national and international practices and is my secret to how I am able to bring all of these innovations and new ideas to you!

Fraxel Dual 

The winter is a great time to address the damage that we have incurred during summer months. The Fraxel Dual is a new non-ablative technology that uses two different wave lengths (1550/1927) to address the pigmentary changes caused by sun exposure. It is also helpful for mild to moderate wrinkles, acne scars, melasma and actinic keratosis. The 1550 aspect of the fraxel treats the deeper pigmentary and surface changes, while the 1927 addresses the more superficial pigmentation. Its technology is very safe, with predictable results. This allows patients to have a great result with minimal downtime and disruption to their daily routine. Patients require one to two treatments for optimal results.

 

ePrime

ePrimeTM is the first minimally invasive aesthetic treatment to improve facial skin laxity and volumize the skin at the same time. The ePrimeTM treatment improves the thickness, firmness and elasticity of the skin with only one treatment.

The device has been shown to give about one-third the effect of a face lift. In case you didn't see it, we were featured in an article in the Michigan Avenue Magazine November 2011 Issue for being one of only 10 centers chosen in the US to pioneer the device.

CoolsculptingTM by ZELTIQTM 

During the winter holidays we all seem to feel the calories pile on. Fortunately, with New Year's resolutions to follow, we often make efforts to take back the reigns and get back on track. For those wishing to jump-start the process, CoolsculptingTM by ZeltiqTM offers a truly extraordinary opportunity to non-invasively provide fat-reduction for particularly stubborn areas. Thermage® 

Two of the most common cosmetic concerns addressed in the dermatology clinic are the increase in skin laxity and the appearance of fine lines and wrinkles due to the passage of time. Unfortunately, through the natural aging process, skin undergoes a gradual loss of collagen. Meanwhile gravity and environmental factors also take their toll on the skin. For those wishing to turn back the clock a few years Thermage® offers an exciting opportunity. By harmlessly and noninvasively heating the deeper layers of skin, Thermage® is able to tighten existing collagen and at the same time stimulate new collagen production. This results in smoother, tighter skin, and a reduction in the appearance of fine lines and wrinkles. A more youthful appearance can be achieved without surgery or injections.

Xeomin® 

Xeomin® is the newest of the 3 FDA approved botulinum toxins on the market in the United States. It has the exact same active molecule in it as Botox® and DysportTM but it does not have the surrounding proteins that these two other preparations have (which are unique to each of them). The advantage of this may be for people who have noticed that their treatments have not been taking as well or lasting as long as those before them. One reason for this may be that our bodies can make antibodies to the proteins around the active molecule. This would mean that Xeomin® is a good choice for anyone whose past botulinum toxins treatments have had diminishing success.

Acne

Despite proper skin care and healthy living habits, many people suffer from uncontrollable acne. It is not only the teenage population, but also very commonly with those in their 20's, 30's and 40's who deal with regular acne breakouts. Acne can commonly affect the face, neck, chest and back. There are varying types of acne and different levels of acne severity. Acne occurs when a pore becomes microscopically blocked by dead skin cells and debris. Acne causing bacteria may then induce inflammation which is manifested by redness, swelling, and the production of pus. Hormonal factors are commonly related to acne. It is important to note that acne does not occur due to lack of proper hygiene or poor skin care. Often more moderate to severe grades of acne can lead to significant scarring if left untreated.

Regardless of age, gender, or type of acne present, Advanced Dermatology offers cutting edge acne management. Available treatments include topical cleansers and applications, oral medications, medical facials, photodynamic therapy, Isolaz treatment, and acne cyst injection. Often various modalities may be combined to achieve skin clearance. Given the wide range of options available to treat acne today, excellent results are the norm. Burns and Wounds 

Burns and wounds can happen at any time. It is important to treat burns and wounds immediately to prevent infection. Even a minor wound can become infected if bacteria are allowed to build up at the wound site. To treat a wound, first wash the wound with running water. This will help to remove dirt and debris. Next, if the wound is bleeding apply pressure with a clean cloth for up to 10 minutes. If the bleeding won't stop, seek care from a doctor. Next, apply Vaseline® or Aquaphor® to the wound and a dressing or bandage. It is important to keep the wound covered to help it heal and prevent further injury to the area. Many people think that airing out a wound is best, but in truth covering and coating a wound is best. The same is true for treating a burn of the skin except for a burn hold the burned area under cool running water or apply cool cloths to the burned area. If a blister appears in the area of the burn, don't break it. Vaseline® or Aquaphor® and a bandage may be applied and Ibuprofen or acetaminophen may be taken to help with pain or discomfort. We often also recommend Domeboro® Compresses (found over the counter at most drugstores and now carried in skinfo®) that is very soothing and also helps to kill bacteria and viruses. To dramatically speed normal wound or burn healing a cream called Cicalfate may be used instead of Vaseline® and Aquaphor® to treat the area. Cicalfate provides an optimal healing environment for the skin and is fragrance, preservative and
colorant-free.
(Cicalfate can be purchased at skinfo®.) Also helpful isTNS® Recovery Complex. TNS® contains growth factors which speed wound healing and can potentially prevent scarring of some wounds. After wounds and burns heal, laser treatments are available to improve the appearance of the skin. Laser treatments can decrease redness or brown discoloration in the skin. Laser treatments can also treat scarring such as pink, red, or thickened scars.

 

Winter Weather Skin Conditions   

As winter approaches Chicago the weather can be very difficult on our skin. There are many skin conditions that are affected by the dry air of the winter months. Cold weather, lack of humidity, and use of fabrics such as wool can all cause irritation to the skin. Eczema or atopic dermatitis, psoriasis, and xerosis or dry skin are all aggravated by the winter weathers.

 

Eczema or atopic dermatitis can flare up, causing the skin to become itchy, dry, and even crack. To help prevent eczema flares during the winter, be sure to use mild and moisturizing soaps, use a thicker moisturizing cream after bathing, and avoid known triggers. A humidifier and keeping your home at a comfortable, but not overly warm, temperature can also be helpful.

 

With reduced exposure to ultra violet light in the winter, psoriasis often flares. Light therapy, such as Xtrac can help clear up the patches and scaly skin of psoriasis when natural ultra violet light is not possible. It is important to be aggressive about moisturizing immediately after luke warm showers and applying your medications as needed to affected areas.

 

Xerotic or dry skin is also exasperated in the dry cold months. Minimize your bathing times, using luke warm water and moisturize not only after bathing, but several times a day to help relieve the affects of dry skin. When moisturizing use a thicker moisturizer such as Vanicream ($15.75). Try to avoid aggravating materials such as wool.

 

For all skin types, not just those with eczema, psoriasis, or xerosis it is important to follow the following steps to keep your skin healthy through the winter months.

1. Limit your time in the shower and try to use luke warm water. Hot showers aggravate the skin.

2. Use moisturizing soaps and bathing gels such as La Roche Posay Lipikar Bar ($10.50) or or Avene Cleanser for Intolerant Skin ($20).

3. Immediately after bathing, dab yourself dry and apply moisturizer like the skinfo® Vital Tea Body Balm ($44).

4. Humidify your home and/or bedroom.

Proper Lip Care for the Cold, Dry Months

In the middle of winter is when we find our lips to be dry, chapped and flaky. These 4 tips can prevent your lips from becoming dried out this winter.

 

1.Don't lick your lips. Repeatedly licking your lips can cause a drying effect similiar to overwashing your hands that can make chapped lips, worse. Instead apply

3.Make sure you have a diet rich in Vitamin A, C, and E.These essential antioxidants can repair, protect, and prevent damage to the sensitive skin. Vitamin A is helps to reduce fine lines and wrinkles and also prevents dry skin. Vitamin C helps tissue to repair itself and recover from sun damage. Also, if you have a lip treatment that contains Vitamin C, this will help to boost collagen production for fuller lips! Vitamin E prevents damage by fighting off free radicals, it is also a great aid to smooth and soften lips.

4. Use a lip balm that contains an SPF whenever you go outside. This is the most important step to prevent your lips from losing hydration and to prevent your collagen from breaking down.

 

Even if you don't do all of these steps everyday, just following a few can make a drastic improvement in the health of your lips this winter!
