Dear patients, friends and colleagues,
I hope you survived our very rainy spring and are now ready to officially enjoy some good weather. This summer's issue will keep you updated on how to care for your skin during the hot season which is when we see many bug bites, heat rashes, sunburns, and other seasonal issues. We are also going to share some information on cosmetic treatments that can be completed successfully throughout the summer months. And, as always, we are eager to share with you news on how we continue to improve the practice.
We are celebrating one whole year with Dr. Emily Rubenstein and Steve Prus, PA-C joining the practice. They are very busy now and are establishing themselves as an important and stable presence in the practice. If you haven't met them, please say hi or make an appointment. They are excellent, committed and energetic dermatology practitioners who share my passion for dermatology and who have outstanding technical and procedural skills.
You have heard much in the news about physicians practices adopting electronic medical records. Most of you know that we have ALWAYS had a computerized office, but we did do most of the visits on paper and then scanned them into our servers. We adopted this method 8 years ago. The software wasn't robust enough for the needs of our practice. Finally, the systems have become sophisticated enough for us to install a new system. The system we chose is called NexTech, and it is used by many of the top dermatology and plastic surgery practices in the country. What does this mean for you?
- Better systems mean better communication not only with you, but with our labratories.
- It means less paperwork on your part AND our part.
- It means we can easily search for everyone on a specific medication if we need to inform people about something.
- Integration of the photography system we use with the chart.
- Being able to tell you where you stand with your deductible and if a procedure or visit is going to be covered.
- Integration of skinfo's records so that we will know exactly what you are using and how it fits together with your prescriptions.
I could go on and on!
We hope that you notice the improvement and know that we want to provide the very best. Our goal is to give you more attention and spend less time with the paperwork! We can't go an entire quarter without new equipment, so this quarter is no exception. The Pellevé™ is a new tightening, brightening and firming system that we now perform in our office. Often people ask me, which is the best treatment for rosacea, sun damage, skin tightening, acne scars and wrinkles? The truth is there is never any one answer. Having access to multiple technologies affords us two very important advantages: we can offer different people different options depending on how much down time they have, what type of skin they have (thin or thick, dry or oily, sensitive or not?) and what their budget and time will allow. Second of all, we can combine different treatments for best results. Combination therapy is always praised at advanced aesthetic meetings as giving the best results. This is the key advantage of Advanced Dermatology. If all you have is a hammer, everything looks like a nail. But because we have a whole set of tools we can adapt...you can be you and we can adapt the technology to your individual situation.
We recently had an event called "Skin for Life" that included a fashion show by 3 Chicago designers, a DJ and a silent auction. We are proud to say that we raised over $3,000 for the Melanoma Research Foundation. Included in this newsletter are a few pictures from that event (see below). Zeltiq™ Diary
What did you expect prior to having your Treatment? I expected to get tighter skin and a more defined abdomen since I workout and was told hat it would help the lower abs which are tough to target with exercise alone. (See before and after photos below of male patient's treatment).
What did you experience?
A little discomfort when applicator was applied and I got a little light headed and then got a little warm for about 2 min max. After that, it was great and then I felt cramping when applicator was removed but that only lasted about 5 min while they massaged the area. Was the treatment painful?
Overall the treatment was relatively painless.
How did you feel post treatment and the next day?
I was slightly sore and tender and numb for about 2 weeks in the area treated. How quickly did you see results?
I got phenomenal results and saw improvement at about 5 weeks. However, I do feel that it is essential to do more than just one treatment to get maximum results.

BOTOX®
Although this is an "old topic", it is worth saying again that injectables are the mainstay of aesthetic practice. The whole idea that you can change how you look "temporarily" and easily, without surgery, was started by Botox® and extended by fillers such as Restylane. They were the originals and are still our mainstays. Many of you have heard that I was an artist and I do think of the injectables and lasers as my palette and my toolbox. But the artistry is at its most with the injectables. It is truly a pleasure to modulate a facial shape and features into one that looks more youthful and which makes that person feel better about themselves. Giving women and even men back their cheekbones often reduces the folds around the mouth and lifts the face visually so that alone makes a big difference in how a person looks. Turning a frown into a smile, a scowl into an open face, deep grooves by the sides of the mouth into a sloping curve, a flattened cheek into a full and youthful one - are the aesthetic pleasures of being a dermatology artist. For those of you who have sat on the sidelines due to fear of toxins and needles, you are missing out! You don't have to look "done", fake, or like a Hollywood starlet or leading man gone wrong. Good aesthetic work goes unnoticed because it is natural. I have spent a very long time trying to create natural appearing facial features with injectables and I believe we have succeeded in pairing the right brands for the right occasions and getting optimal and natural looking results. If needed, we even have a laser that will cut your bruising by 50%, which is a big advance and benefit.

Hyperhidrosis
Hyperhidrosis is a condition characterized by abnormally increased perspiration. It commonly affects the underarms, palms, and soles but any part of the body can be involved. Hyperhidrosis can be classified into two types, Primary and Secondary. In Primary hyperhidrosis the cause is often unknown. The excessive sweating usually starts in adolescents and can worsen in puberty. Primary hyperhidrosis often affects the underarms, palms or soles. Secondary hyperhidrosis is less common than primary hyperhidrosis and is caused by another medical condition such as hyperthyroidism. Secondary hyperhidrosis often affects the entire body and can be controlled by treating the underlying medical problem. Primary hyperhidrosis can be effectively managed and numerous treatment options are available. First, topical agents such as prescription strength antiperspirants can block sweat glands. Oral medications such as anticholinergic drugs can also diminish sweating. Lastly, injections of botulinum toxin type A, such as Botox, can be used to disable the sweat glands. With so many effective treatments available it can be easy to manage hyperhidrosis.
Folliculitis
The approach of warmer weather tends to increase our outdoor activity level. With increased exercise and perspiration, friction from clothing and sports equipment, as well as increased shaving, some individuals will find an increase in irritation to their hair follicles. Folliculitis is the medical term for irritation or inflammation of the hair follicle. Folliculitis is a very frequently addressed problem at Advanced Dermatology.
Any hair-bearing body area can be affected by folliculitis. When irritated, the follicles will appear swollen and red, and may be itchy or painful. Sometimes small pustules can form. Usually folliculitis is limited to one or two body areas, but occasionally it can be more widespread. This is usually only the case when bacteria are the cause.
There are many treatments available for folliculitis. The body site and the suspected cause must be considered when choosing how to treat. If bacterial involvement is suspected an oral or topical antibiotic is often used. For folliculitis based upon irritation from friction, equipment, or razors, medications and products designed to ease irritation and condition the skin are utilized. These may be prescription drugs, cosmetic products, or various combinations of the two.

Differences in Sunscreens
With so many different types of sunscreens on the market it is difficult to know which sunscreens are the best. Sunscreens now come in a dizzying array of sprays, lotions, foams and powders and in a wide range of SPF's from 15-100. It can be confusing for even the best educated consumer to pick the most effective sunscreen. Most of us know a sunscreen with an SPF of 30 or higher is a good place to start. However, the SPF doesn't tell the whole story. What we need to focus on are the product's ingredients because the ingredients are directly connected to the level of protection. There are two basic types of sunscreens, chemical sunscreens and physical sunscreens. Chemical sunscreens protect our skin by absorbing UV rays. Some ingredients commonly used in chemical sunscreens are avobenzone, oxybenzone, and octisalate. While these types of sunscreens protect against UVB rays and short UVA rays, they don't protect against long UVA rays. Long UVA rays can cause wrinkles, brown spots and skin cancers. Physical sunscreens protect our skin by creating a barrier on the surface of the skin which reflects the UV rays. Some ingredients commonly used in physical sunscreens are zinc oxide or titanium dioxide. These types of sunscreens block UVB, short UVA and long UVA rays. Because physical sunscreens block all of the sun's harmful rays, they provide the best protection. Therefore, the most effective sunscreens available contain an SPF 30 or higher and zinc oxide.
Using a physical sunblock with Zinc oxide is not only important to protect against skin cancers it is also important for patients with conditions that are affected by UV rays. There are several different skin conditions that can be aggravated by sun exposure.
One such condition is called polymorphous light eruption (PMLE). PMLE is a common, sunlight-induced eruption affecting individuals of all races. Attacks are intermittent and follow minutes to hours (rarely days) of exposure of the skin to sunlight or artificial UV exposure such as tanning beds. Non-scarring, itchy, red bumps, blisters and/or plaques then develop minutes to hours later. The eruption is generally most severe in the spring or early summer, and it usually disappears completely during the winter.
Another condition that can is actually caused by sun exposure is Drug-Induced Photosensitivity. Photosensitivity is characterized by an exaggerated sunburn. The following mediations are commonly associated with this condition, NSAIDS (used primarily to treat inflammation, mild to moderate pain, and fever), certain diuretics, certain antibiotics and anti-fungals. Treatment of this condition is sun protection.
A common condition we see as spring break concludes and summer vacation begins is Phytophotodermatitis. This is a phototoxic reactions consisting of redness (with or without blistering) and delayed darkening of the skin. The combination of a certain sensitizer and UV light causes this reaction. The most common sensitizers that patients come into contact with are citrus fruits such as lemons and limes, contact with certain flowers, weeds, and vegetables such as parsnips, parsley and even celery. Avoidance of such sensitizers is the best way to avoid these conditions. In the summer be careful drinking beverages with lemons and limes, as there juices can come into contact with the skin.
Clarisonic
Sonic cleansing removes 6 times the dirt, oil and makeup than regular cleansing alone which allows products to penetrate the skin 60% better! Has been published in journals and has clinicals behind it to show that it really works! Sonic cleansing can be used on the most sensitive skin since the brush vibrates over the skin.
Over the counter
The brush actually rotates which pulls on the skin and causes irritation. There have also been reviews stating that their hair was caught around the brush! There aren't any clinical studies behind the product either.

Advanced Skin Lightening Technology
Elure™ is an exciting new skin lightener that has taken Asia by storm. The #1 complaint of Asians is uneven pigmentation of the skin. Here we try to be tan, there they try to be lighter! They tend to get few wrinkles but more brown spots. Indian and middle Eastern skin tends to get patchy with different color shades as can African-American skin. Caucasian skin can get sun freckles and also can get uneven skin or melasma. All of these conditions are difficult to treat with both lasers and creams.
