Dear patients, friends and colleagues,

This winter has been a challenge. In our typical Midwestern fashion we got through it. There was that one week in February when everyone had just had it (the week after the blizzard). Now we are happily looking forward to spring and the hope that we can go outside and have more freedom.

Many of you have taken the time to respond to our e-surveys after your visits. We appreciate it and we hear you: you want easier ways to communicate with us. We have introduced RelayHealth, an online secure method of communicating with you via the internet and email for general questions (no emergencies), appointment and/or refill requests. For full details see "You can now communicate with us ONLINE!" below.

We also know it can be difficult at peak times to get through to us on the phone. We have added additional client service staff members to help us improve our communications. We will continue to expand our call center to more individuals so that you can communicate with us in a time-appropriate manner.

We are still abuzz with our new technologies, the Fraxel Dual laser and the ZeltiqTM Coolsculpting Device. The Fraxel Dual laser gives us the opportunity to significantly eradicate most brown spots and sun damage as well as very superficial wrinkles in 1-2 treatments. This is a huge advancement in our field. In addition, this treatment has been shown to be very effective for Actinic Keratoses (AK=pre skin cancer) (and in fact is approved by the FDA for this treatment). Although it is not covered by insurance, this treatment can eradicate years of sun damage and could reduce significantly the number of visits and treatments for AK and recurrent AKs. (See below for more details under "new options for AK"). Although it has not been shown to cure AKs, it is certainly a great option for those who have been frustrated over the years with multiple therapies and visits for AKs. In addition, late last year we added two significant pieces of high technology equipment; the V-Beam laser, which targets blood vessels and rosacea, as well as the Quadralase which is a CO2 fractional resurfacing laser. See below under "New Technologies at Advanced Dermatology".

Under our Medical Dermatology section we highlight some of the advances in psoriasis care (see below). The Fraxel Dual

 We upgraded our Fraxel in December from the "Restore" to the "Dual". What does this mean for you? In addition to the remodeling benefits of the skin that we could achieve with the Restore, we can really eradicate most Photodamage in 1-2 treatments with the new wavelength, the Thulium. (See Before and After Photo in this newsletter). The Dual allows us to treat the top layer of skin where the brown sun freckles and age spots are, as well as the dullness that comes with age and sun exposure. Even superficial wrinkles can be improved significantly with this treatment. Best of all, rather than 3-5 treatments, benefits are achieved in as few as 1-2! Besides the face it can also be used on the chest, the hands, arms, legs and even back. There is some redness and peeling with this procedure that can last up to a week. New options for Actinic Keratoses 

As you know, Advanced Dermatology was a pioneer in the treatment of photodynamic therapy for actinic keratoses. But, there are a couple of new options for you to consider as well.
The Fraxel Dual (see above) was recently approved by the FDA for the treatment of actinic keratosis. This laser can achieve a very dramatic change in the skin with a treatment for both the epidermis (top layer) and the dermis (middle layer) of your skin. This is important in that when we only treat the top layer of skin for precancerous cells there is often only a 1 to 2 year period before new abnormal cells recur. This may be due to a chemical signal coming from damaged cells in the middle layer of the skin. It may be possible to have longer lasting global improvement by treating both areas, which is accomplished with Fraxel Dual.


During a six month clinical study of 21 subjects at several dermatology centers around the country, there was an 83.5% average reduction of AK lesions using the Fraxel restore Dual's 1927 nm wavelength. Treatments were shown to be effective both on and off the face, including the arms, hands and chest area. In our study, we found the Fraxel Dual to be the most effective in the treatment of multiple facial AKs. "This treatment is safe and requires minimal downtime while simultaneously improving other signs of photodamage," said Roy G. Geronemus, MD, Director of the Laser & Skin Surgery Center of New York. "While the 83.5% removal rate was comparable to other topical therapies and AK treatment options, Fraxel was overwhelmingly well-tolerated by all the patients and offers the added benefit of improving a patient's' overall skin quality, color and texture."


Don't forget to ask about all options when seeing your provider for treatment of your AKs. Although the Fraxel Dual treatment is FDA-approved for AK treatment, it is NOT covered by insurance, it is your choice to make treatment decisions and sometimes we are reluctant to bring up treatment options that require you to pay for your medical treatments.


Zeltiq Diary
What did you expect prior to having your Treatment?

I was really excited to try this new technology out (I could use all the help in the abdomen area that I could get). I was not really sure what to expect because I was the first one to have it done here.


What did you experience?

The first 10 minutes were a bit uncomfortable. The suction was a lot more than I had thought it would be. Once the area was cold I did not have as much discomfort. I was able to relax and study for my finals after the first 10 minutes or so. Once they removed the applicator, my lower stomach was like a frozen brick, it was kind of cool. My stomach was cold in that area for several hours after the treatment.


Was the treatment painful?

I would not say the treatment itself was painful. A bit uncomfortable at first but after about 10 minutes I was used to it and relaxed.


How did you feel post treatment and the next day?

Post treatment my stomach was cold for several hours. I didn't have much discomfort right after the treatment. The next day and for several days following the treatment I was sore. I was unable to work out for 3 days because my stomach was sore and running was painful.


How quickly did you see results?

I started seeing results 4 to 5 weeks after the treatment. I was sore for about 2 weeks after the treatment. Some days were worse than others, but it was tolerable. I was unable to sleep on my stomach for a week because the area was just too sore. I am now in my 9th week post treatment and have seen fantastic results! I am very happy with the treatment! It was worth the few days of being sore and a bit uncomfortable.

Biologics and XTRAC for Psoriasis 

While we look ahead to the warmer spring days, we must continue to manage the negative effects that cold and dry conditions have on our skin. Unfortunately, many people have skin issues that tend to worsen with the colder seasons. Individuals with psoriasis may be particularly impacted. Psoriasis is a genetic condition involving the immune system which causes skin rashes and in some cases joint pain and swelling. Most commonly the rash appears as raised, dry, scaly patches on the skin. Any area of the body can be affected, and severe itching can often accompany the rash. The exact cause of psoriasis remains unknown, and there is no known cure.

 

Fortunately, over the past decade, our treatment options for those suffering with psoriasis have greatly expanded. Now, even in the most difficult cases, dramatic improvement of psoriasis can often be achieved. In fact, with treatment many of our patients will see their psoriasis plaques clear more than 75%. Our management of psoriasis has been revolutionized by the introduction of what are referred to as "biologic medications". These include Enbrel, Humira, Remicade and Stelara.

 

The biologics work by blocking specific components of the immune system that are responsible for the skin rashes and joint inflammation due to psoriasis. Unlike many older medications, the biologic medications are given by self-injection, in-office injection, or IV infusion. Their onset of action is usually within the first three months, and is enhanced by use of topical medications at the same time. People who use biologic medications generally maintain excellent results over long periods of time. With high safety levels and amazing efficacy, the biologic medications have truly changed the lives of many people. As with all medications, there are potential side effects that we will discuss with you in a medical visit to determine if these medications would be right for you.

 

At Advanced Dermatology, we also treat psoriasis with the XTRAC laser. Approved by the FDA for the treatment of psoriasis in 2000, the XTRAC laser is a very safe, painless and an effective targeted approach to psoriasis treatment. It utilizes a narrow-band ultraviolet light source to target specific areas of skin affected by psoriasis, while leaving unaffected areas untreated. Treatment sessions are usually scheduled twice weekly. XTRAC laser treatment is a great option for someone who doesn't wish to use topical or systemic medications, or someone who is currently treating with medications but experiencing an inadequate response to treatment. In most instances, medical insurers cover the cost of XTRAC laser treatment. We will preauthorize the treatments with your insurance company and discuss with you, your out of pocket expenses before you begin therapy.

With many safe and effective treatment options available, successful management of psoriasis has become achievable for the vast majority of patients.

 

Wart Treatments  Warts are non-cancerous skin growths caused by a viral infection in the top layer of the skin. Warts are passed from person to person, either from skin to skin contact or through contact with the virus on a fomite (the floor, the doorknob, etc), and can occur almost anywhere on the body. We offer a wide variety of successful treatments for warts and are able to customize treatments for each patient. The treatments for warts include prescription creams applied to the warts daily, an oral pill to enhance the immune system's ability to fight off the warts, freezing the warts with liquid nitrogen or an injection of medication into the warts. There are two different medications which can be injected into warts. One medication is an anti-cancer drug called Bleomycin. This works because the wart virus makes the skin cells it inhabits grow quickly and cancer drugs target quickly growing cells. Since a very small amount of the drug is used and it is injected locally and superficially, the drug does not have any internal effects. The other medication is a drug called Candida which stimulates an individual's skin immune system when injected into the wart. With each treatment the warts will become shallower and smaller in diameter as the virus causing the warts is destroyed by your own immune system. The bonus of this treatment is that it also makes less likely for a recurrence of the warts, comparable to receiving a vaccination as well as a treatment. While these treatments are usually successful, each method does require numerous treatment sessions and sometimes combinations of treatments over weeks to months before the warts are completely resolved. It is not uncommon for warts to take three to five treatment sessions before they are gone.

 

Spring Fever Sun Protection  With spring break quickly approaching it is time to remember the importance of sunscreens and review their proper use. We don't want anyone to be left out in the sun without proper protection. First, it is important to wear sunscreen because the sun gives off ultraviolet (UV) radiation. The UVA and UVB rays given off by the sun can damage the skin. This damage leads to signs of premature aging in the skin as well as skin cancer. Wearing sunscreen can prevent much of this damage. When choosing a sunscreen, it is important to choose one with a SPF 30 or higher and with the ingredient zinc oxide included. Zinc oxide provides the most effective and immediate protection from both UVA and UVB rays. There are numerous cosmetically acceptable zinc oxide sunscreens on the market including non-greasy lotions, sprays and even powders. Check out the wide range of sunscreens with zinc oxide carried at skinfo¨ and you'll be sure to find one you love. Most likely you will want different sunscreens for different situations: on the beach, everyday and when out and about. Once you choose a sunscreen, it is important to apply it daily to the areas of the body which are exposed. And, don't forget to reapply! Reapplication of sunscreen should be done every 1.5-3 hours and more often if you are swimming or sweating. Have fun this spring break but make sure to care for your skin by protecting it daily with sunscreen.

 

Bowen' s Disease  Bowen's disease, also known as squamous cell carcinoma in situ (SCC in situ), is a form of skin cancer. The term "in situ" tells us that this is a surface form of skin cancer. Bowen's Disease was first described by Dr. John T. Bowen in 1912 and thus bears his name.

 

Bowen's is usually a red, scaly patch. It tends to be seen on areas frequently exposed to the sun such as the face, arms and legs. Some itch, crust or ooze, but most have no particular feeling. SCC in situ may be mistaken for rashes, eczema, fungus or psoriasis. Sometimes they are brown and look like a benign keratosis or a melanoma. Because of this, a biopsy must usually be done to confirm the diagnosis.

 

Causes of Bowen's include sun damage, exposure to arsenic, immuno-suppression (cancer drugs, drugs for certain types of psoriasis, arthritis, Crohn's disease, HIV), certain viral infections, and chronic skin injuries. Bowen's disease rarely occurs before the age of thirty and most commonly occurs in individuals over the age of sixty. Interestingly, women outnumber men in this diagnosis, with 70-85% of the diagnosis occurring in women. Of these between 60-85% occur on the lower leg.

 

The simplest and most common treatment for SCC in situ is surgical excision. Other options of treatment include curettage and electrodessication (ED&C) which is essentially a burning and scraping of the skin cancer, as well as the use of photodynamic therapy (PDT). PDT is an alternative way to treat SCC in situ using a drug that is absorbed preferably by cancer cells. A bright light is then applied causing the release of toxins and destruction of the tumor.

 

Advanced Dermatology specializes in PDT. PDT is a preferable option because it can leave less scarring. However, it is not approved by the FDA in the US (it is widely used in some European countries for this condition). We can discuss treatment options for you if you are diagnosed with this condition.

 

If you have had an SCC in situ, you have a higher risk of other skin cancers. Untreated, SCC in situ grows larger over time and may spread out to be several inches. 5% of SCC in situ will eventually develop into invasive squamous cell carcinoma if not treated.  Prevention is always the best way to "treat" diseases! Don't forget to apply sunscreen to your lower legs, front AND back as well as schedule annual body checks.
