Winter January - March 2011
Adv Derm Header
Dear patients, friends and colleagues,

It's that time of year again for us to hibernate and try to stay warm and keep our attitudes upbeat, keep our skin moisturized and if we are lucky, visit warmer climates for a few days (while using our sunscreen, naturally!)

We are having a special issue of our newsletter to reintroduce you to our providers. I am very certain that this is a great lineup of skilled, caring and personable providers, and that you can get the best medical, surgical and cosmetic dermatologic care here at Advanced Dermatology and Skinfo.

We are also thrilled to announce the addition of the Zeltiq device to our service offerings. The Zeltiq was recently approved with huge introductory publicity and many of you asked me whether we were going to adopt this new noninvasive technology for removing fat. I wanted to be certain that the device was safe and effective before purchasing it, but after reviewing the available literature and discussing with colleagues all over the country I have determined that this device is a true advance in our field. There will be more about it in the issue below. For anyone with an unwanted pocket of fat that is immune to diet and exercise (and who among us do not have at least one area that fits that description!), I believe Zeltiq gives us a shot at noticeable improvement with minimal discomfort or downtime.

I always enjoy going to Orlando Dermatology Aesthetic and Medical Conference. This will be the 8th annual meeting where I have been a speaker since its inception 8 years ago. Now, on the organizing committee, I have made friends from all over the country and met countless residents undergoing training. I still love to meet young doctors who are excited and enthusiastic about dermatology. I never forgot how I "fell in love" with dermatology, no matter how geeky that may sound, during my first rotation in medical school and still feel that same passion today.

One way to get ready for spring is to make up your mind to make your skin healthier and this is a good time to do it. Come in and enjoy our new office wifi for your convenience and we also have an iPad for you to check out at the front desk if you are sitting with anesthetic cream on, getting a Zeltiq or just want to be entertained.

Have a great winter - get on some ski clothes and take a walk (I hear groans, but you will work up a sweat!)

Dr. Taub
Best Regards,
Amy Forman Taub, MD

Amy Taub is the founder of Advanced Dermatology, Skinfo and SKINQRI. She grew up on
the North Shore (Wilmette and Glencoe), went to New Trier West High School and graduated from Kansas City Art Institute with a BFA in painting. After moving to New York City and finishing one year of a master's degree in art, she decided to pursue pre-medical studies at Columbia University. She then returned to Chicago for the rest of her training, completing medical school, internship in Internal Medicine, and Dermatology Residency at Northwestern University Medical School and its affiliated hospitals in 1989.

She initially was in practice in a single specialty dermatology practice located in Barrington and Crystal Lake, IL. After 9 years, she relocated and become the first Director of Dermatology at Northwestern Memorial Physicians Group, a large multi specialty group based mainly in the city.
She spent 5 years with them, opening a new office in Northbrook and helping them to build their
dermatology practice from 1 provider to over 6. This practice now has over 10 physicians in 3
locations.

Always looking for a challenge and wanting to deliver her vision for dermatology, she opened Advanced Dermatology in February, 2003 with one doctor (herself), one esthetician and one Physician Assistant (whom she trained for one year before she saw any patients). (The first day the office was open we wondered how we would walk up and down that long hall every day and fill up all of the space!).

Since that time she has authored 16 peer reviewed publications, 5 book chapters and spoken at multiple national meetings on a variety of topics including procedural treatments for acne, photodynamic therapy, filler treatments, laser resurfacing and multiple different laser modalities, radiofrequency devices and many others. She is considered a thought leader in esthetic and procedure based dermatology and is a consultant to many leading laser, esthetic and medical
dermatology companies. In addition, skinfo is one of very few physician-associated dermatologic
skin care boutiques that carry a multitude of product choices and employ highly trained certified
estheticians to help people select their individualized skin care.

She still has a passion for dermatology as she never tires of the satisfaction of making people feel and look better.

AestheticnewsNEW ZELTIQ Cool Sculpting Fat Reduction Device  The CoolSculpting by ZELTIQ procedure can reduce the fat layer by 20 to 40%. This can be used on the abdomen ("muffin top"), back fat or love handles). More than one treatment and 3-6 months may be necessary for optimal results. The provider will mark the areas to be treated and the cooling panels will be positioned on the areas. The device then draws the bulge up with suction. The sensation is a firm tug and pull. In the first few minutes, there is a feeling of intense cold that soon dissipates. The patient is seated comfortably or lying down during the one- to three-hour procedure. Many people spend this time reading, working on their laptop, or even taking a nap. The CoolSculpting procedure is completely noninvasive, so the patient can return to normal activities immediately. Some patients experience redness, minor bruising, tingling, or numbness in the treated area, but this is usually temporary. After the Coolsculpting procedure the treated fat cells are eliminated slowly for a few months. As long as the patient maintains his or her weight with diet and exercise, long-term results should remain stable. Please contact our office for a consultation for this new revolution in body shaping!

The eMatrix™: Sublative Rejuvenation  Dr. Taub, as many of you know, helped the company Syneron to develop their "eMatrix™" device. Now in its third generation, the eMatrix has 2 new applications: surface pigmentation and wrinkles around the eye. The eye area is very delicate and also susceptible to damage from laser treatments due to the fact that light can penetrate into the eye. For laser treatments we must use eye protection which interferes with being able to reach the eyelid skin directly under and over the eye. Radiofrequency doesn't go through the skin to the other side so we don't need to cover the eyes. We can improve the hooding of the upper eyelid and the wrinkles of the lower eyelid more easily. Best of all we have had no serious complications with this device. We can use it for ANY skin type, no matter what color, tanned or untanned and even with acne breakouts present to help with acne scars. We improve hooding of the upper eyelid and fine wrinkles of the lower eyelid. The Sublative rejuvenation procedure decreases wrinkling as well as scars and tightening and brightens the skin of the face, chest, neck and arms more quickly and with less overall down time than other resurfacing lasers and best of all it is has been VERY safe in our hands, with an extremely low few adverse event rate.

Triniti™ Effect: PR, Skin Tightening, Wrinkle Reduction  The Triniti is an aesthetic skin treatment program which delivers an overall refreshed and more youthful appearance with minimal downtime. The Triniti™ program is designed to reduce redness and brown pigment from the skin, tighten loose skin, and reduce the appearance of wrinkles. This is achieved by combining three of today's most effective laser technologies, FotofacialRF®, Refirme™, and Matrix IR™, into one treatment. This is accomplished by performing each of these three treatments sequentially in the same office visit. The Triniti™ offers a noninvasive treatment for total facial renewal.

Effective Wrinkle Erasers: Botox/Dysport, HA Dermal Fillers; Matrix; Quadralase  Advanced Dermatology offers many different types of treatments that can effectively diminish the appearance of wrinkles.

Botox and Dysport are injectable products used to temporarily improve the look of frown lines between the eyebrows. They work by stopping the signal from the nerve to the muscles, which results in a reduction of muscle activity and temporarily prevent contraction of the muscles that cause frown lines. Treatments are recommended every 3 to 4 months.

Injectable Fillers to enhance volume, erase wrinkles, augment lips, and overall restore the youthful 3-demensional contour of the face. The most common fillers used contain hyaluronic acid, a naturally occurring human protein responsible for cushioning the skin (Restylane, Perlane, Juvederm). Treatments are recommended every 6 to 9 months. Filler injections require great skill and artistry and we pride ourselves on achieving natural looking results.

If you are a person who can spend a few days out of the limelight and want quick results without multiple treatments, The QuadraLASE fractional laser resurfacing is a great choice for you. This laser removes layers of skin tissue in a fractionated method (by leaving surrounding tissue intact) to help reduce the components of sun damaged skin such as "age-spots", fine lines, wrinkles, uneven coloration, skin laxity, and textural irregularities. Usually only one treatment is required because the results are dramatic. Healing typically takes place over 5-14 days, depending on the aggressiveness of the treatment and the depth of the changes desired.

To determine what type of procedure is best for you, call to schedule an appointment with one of our cosmetic coordinators!

Precancerous Lesions Actinic Keratosis and Dysplastic Nevus  There are several skin conditions
that can be a "precancer" or an indicator that one may be prone to skin cancers. Two of the most
common are known as actinic keratosis and dysplastic nevus.

An actinic keratosis also known as an
AK is usually a red or flesh colored scaly patch or bump that arises on the skins surface. Actinic keratosis can be the first step in the development of skin cancer. It is estimated that 10 to 15 percent of active lesions, which may be thicker, redder or more tender than the rest will take the next step and progress to a squamous cell carcinoma. Fortunately, these cancers are usually not life threatening, provided they are detected and treated in the early stages. However, if not discovered in a timely fashion, they can bleed, ulcerate, become infected, or grow large and invade the surrounding tissues, eventually spreading to the internal organs.

Dysplastic or atypical nevus (mole) can be flat or raised, and range in color from pink to black. They are concerning as melanomas may arise in them and they can be an indicator that one is more prone to getting melanoma.

Common ways to remember changes that are seen in melanoma are the ABCDE's of melanoma.

  • Asymmetrical lesion
  • Border of the lesion is irregular
  • Color: there is multiple colors or changing colors
  • Diameter: moles greater than 6mm are concerning (but some less than this are also)
  • Evolution: evolving moles or changing moles are more likely to be atypical
Many moles, asymmetry or change in a mole or just a mole that looks different from your other moles should prompt you to come in to get a body check. The National Institute for Health says you should get a body check at least every 3 years even if you don't have any risk factors.

Fighting Winter Skin Issues (Dry Skin, Eczema, Psoriasis)  Our Midwestern winter weather brings a substantial decrease in both temperature and humidity compared to what our skin is accustomed to during the warmer seasons. Our skin provides a natural barrier against drying and irritating external elements, but our skin's resources can quickly and easily be overcome by harsh winter conditions. It is now more important than any other time of the year to assist our skin in maintaining its barrier function. This can be achieved by providing both increased moisture to our skin and avoiding situations that can overburden our skin's ability to protect itself.

A nice hot shower may feel great on an icy winter morning but when we stay in too long and use drying or fragranced soaps we are compromising our skin more. Try to keep the temperature a bit more moderate, don't stay in the shower too long, use fragrance free  mild soaps and MOISTURIZE even before dry to capture some of the water droplets on the skin. We offer several high-potency facial and body moisturizers at Skinfo that will serve to enhance your skin's natural ability to protect itself. Also, be sure your home humidification system is working properly this winter. Your skin will appreciate it!

Certain people in particular
can be especially prone to skin issues during the winter months. Many families share a genetic tendency for eczema, asthma, hay fever, and allergies. Individuals in such families may have a more difficult time with the cold, dry winter months. Their skin's condition can easily degenerate from being simply dry to becoming red, itchy, inflamed, burning, flaking, and scaling. These are signs and symptoms of eczema. Any body area can be involved, and an untreated flare can potentially last all winter long.

Similarly, many people suffer through the winter months with another skin condition called psoriasis. In such individuals, the skin may be very itchy and thickened and a silvery scale can develop over involved areas. People with very mild psoriasis may have clear skin during the summer months, but those with more moderate to severe psoriasis often deal with symptoms throughout the year. Fortunately, in the past ten years, treatment options for psoriasis have evolved to such a great extent that even the most difficult cases can generally be treated very successfully. The newer biologic agents have changed many people's lives dramatically in the past few years.

If you find your skin is overly dry and irritated or you feel that you may be dealing with eczema or psoriasis, please feel free to schedule with one of our dermatologic providers at Advanced Dermatology for a skin evaluation. We would be very happy to assist you and your skin survive the tough winter conditions. Aside from expert advice, there are many prescription and non-prescription treatment options available to help you deal with dry irritated skin, eczema, or psoriasis.

Research Header
SKINQRI Thank you for your support and involvement in clinical research at SKINQRI to improve treatment options for dermatology patients. Although we have been successful with our research endeavor, I have decided to significantly decrease the amount of time that we are going to devote to clinical research for pharmaceutical and medical device companies. The main reason is that the time commitment in performing research impacts too significantly the time I can spend on patient care and continuing to keep Advanced Dermatology running on a path of continuous improvement, as well as continuing to strive to be on the cutting edge. Fortunately, this decision will create more available resources for Advanced Dermatology to provide continued excellence in patient care.

I want to express my sincere apologies for being unable to perform these trials as they are of great interest to you, me, and the dermatology community. However, we will be performing periodic investigator initiated clinical trials and we may take on certain selected other clinical trials in the future. We also sometimes conduct internal staff trainings for which we need participants. Your contact information will be saved to our database for potential future opportunities of this nature. along with our training database for future staff training days at Advanced Dermatology. On behalf of SKINQRI, Advanced Dermatology, and skinfo®, I want to thank you again for your continued support and patronage.

In light of this transition, we want to again congratulate Ryan Lombardo, SKINQRI's Director of Business Development, on completing his doctorate this Fall. Dr. Lombardo will be opening a wellness center in Evanston this Spring focused on Acupuncture & Oriental Medicine, Nutrition, Prevention, Longevity Medicine, and other Wellness based therapies. The new practice is called Five Phase Wellness Center and is located at 708 Main Street in Evanston. We are very excited for his new endeavor and look forward to the success of his new practice. Please join us in congratulating Ryan on this remarkable achievement.

If you would like more information or have any questions for Dr. Lombardo, please email him directly at acuhealthdoc@gmail.com and view his website at www.acuhealthassociates.com. Remember to sign up for his monthly newsletter to receive healthy lifestyle tips and learn more about the new wellness center!
