 Dear patients, friends and colleagues,
We are pleased to announce that Emily Rubenstein, DO, and Steven Prus, PA-C have joined our practice late this summer. Dr. Rubenstein is from the North Shore of Chicago, we welcome her home. She earned her Bachelor of Arts from University of Michigan, her Masters Degree in Science from Touro University and Doctor of Osteopathic Medicine from Midwestern University. She then went on to complete a Family Practice Residency at Broward General Hospital in Florida in 2004. After becoming board-certified in Family Practice, Dr. Rubenstein then completed a Dermatology Residency at Nova Southeastern where she was Chief Resident and also worked closely with many of the nationally known faculty from University of Miami School of Medicine. Dr. Rubenstein is a bundle of energy and personality as well as a skilled surgeon who is preparing for membership in the American Society of MOHS surgery. We are hoping to offer this service within the next year.
Steven Prus is a Certified Physician Assistant who joins us with over 16 years of healthcare experience. For the past ten years, Steve has practiced general dermatology at a busy general and cosmetic dermatology practice with offices located in downtown Chicago and Kankakee. He brings with him extensive experience in all aspects of medical and surgical dermatology. After receiving his Bachelor of Science degree in pre-medicine and clinical psychology from the University of Illinois at Urbana-Champaign, Steve completed his Physician Assistant studies at Albany Medical College in Albany, New York in 2000. There he received the Julia L. Freitag award, which is awarded annually to the graduate attaining the highest level of academic and clinical achievement. Steve was born and raised on Chicago's northwest side, where he currently resides with his wife and their three daughters.
I am truly excited about the quality of our provider team who are exceptionally skilled in diagnosis, treatment, and dermatologic surgery as well as talented in our aesthetic vision and treatment methods. I am proud to host such a personable and caring crew including Dr. Rubenstein, Dr. Fine, Emily Oehler, PA-C, Steven Prus, PA-C and our esthetic providers Alla Molchanov, Lic. Cosmetologist and Mari De La Roca, Lic. Esthetician.
We do have some very sad news, however. Dr. Garretson has decided to practice closer to the city. Her last day at Advanced Dermatology will be October 24. We will miss her. As our inaugural cosmetic fellow, I thank her for her input in helping us better develop the fellowship as well as her excellent patient care. We all wish her the best of everything life has to offer and continued success.

 Tattoo Removal At some point in your life, you may have felt it was a great idea to have a tattoo on some area of your body... but as you age, many seek out methods to remove the tattoo(s). Whether it is due to the shape changing, the colors fading, or that you just don't like it anymore, tattoos can be minimized if not completely removed from the skin. Successful tattoo removal is a result of "quality switched laser" also known as the Q-Switched laser. The Q-Switched laser delivers short & high power pulses, effectively breaking apart darker colors that are able to be absorbed by the body's immune system. Why is this important? This laser will leave the surrounding tissue intact unlike other methods of removal, allowing for better healing and blending to the surrounding tissue. Points of interest for all tattoo treatments:
- Color does matter: Certain lasers remove certain colors. Also, keep in mind the lighter colors are more difficult to completely remove.
- Treatment quantity: Treatment quantity varies upon patient satisfaction; average for most removals is 3-10, provided every 1-2 months.
- As the old saying goes: "No pain, no gain": Just as it was uncomfortable to receive the tattoo, there is also discomfort with the removal. To reduce discomfort, topical or local anesthetic is often used just before the procedure, in addition to very cold air blown on the area during treatments.
- Aftercare: Just as you had to care for your skin after receiving the tattoo, it is important to also use proper skincare as directed by your laser provider after each procedure to remove the tattoo for proper healing.

Skin Rejuvenation Fall is the perfect time to rejuvenate your skin with light and laser therapy. Time and time again, as much as we try to prevent sun damage, it is inevitable that the visible signs of sun damage will eventually hit the skin surface. For us Midwesterners, this is especially notable after the summer months. With such a high reoccurrence of sun exposure each summer, we develop unwanted fine lines, wrinkles, and red and brown colored areas on our face, chest, hands and even other body areas. Light and laser therapies can safely reduce or remove these concerns with little or no down-time!
The most commonly sought after treatment is Photorejuvenation. Photorejuvenation is on average a series of five treatments designed to reduce unwanted brown and red pigment, while also improving the surface texture of the skin. We choose from three different light sources depending on skin type and diagnosis, sometimes combining technologies for the most effective result. We have recently added in an additional laser to our array of Photorejuvenation devices, the Vbeam Perfecta.
Vbeam Perfecta is a pulsed dye laser designed to improve red blotchy or discolored skin, "broken" blood vessels, rosacea and localized pigmented spots. Pulsed dye lasers also provide benefits to improve skin texture and tone to restore a youthful appearance. The Vbeam will decrease the number of treatments to reduce redness associated with sun damage or rosacea in most individuals from 3 to 5 treatments; whereas a medical client looking to remove deeper lesions such as port wine stains may expect 8-10. This is a non-invasive procedure resulting in no downtime. Skincare and in some cases makeup will be immediately applied after treatment to cover any pinkness so that you feel confident returning to your normal daily activities.
To accelerate the photorejuvenation process while also remodeling the texture (lines, scars, wrinkles) of the face, neck and chest, we have also added QuadraLASE™. The QuadraLASE™ is a carbon dioxide fractional resurfacing laser, one of the most advanced technologies to revitalize skin. This technology removes layers of skin tissue in a fractionated method by leaving surrounding tissue intact.
The skin responds by immediately resulting in new collagen growth and a reduction in the components of sun damaged skin such as "age-spots" (also called sun freckles, liver spots, and lentigines), fine lines, wrinkles, scarring, uneven coloration, skin laxity, textural irregularities, dull tone, and a thinned collagen layer. The number of treatments is typically 1 with a second if needed 6-12 months later. Because this treatment is more comprehensive, there is typically 7-10 days of manageable downtime, depending on the depth of the treatment. The end result is absolutely rewarding with improvement in the skin's tightness, vibrant texture and glowing even complexion.

 | Insurance 104 When is a procedure considered cosmetic? Insurance companies rely on physicians to submit claims on procedures that in their opinion are medically necessary to be performed. Cosmetic procedures could be, but not limited to benign moles, skin tags, seborrheic keratosis and acne/milia extractions. Laser treatments to improve appearance or to treat acne are also considered cosmetic and have to be paid at the time of service.
Psoriasis Psoriasis is a chronic skin disease which commonly causes red scaling patches on the elbows and knees. It can occur anywhere on the body and ranges in severity. For years psoriasis was frustrating to treat due to a limited amount of treatment options. However, we now have a wider variety of treatments available in our armamentarium. The newly improved XTRAC Velocity Excimer laser is a safe, fast, and effective way to improve psoriasis without any pain. Biologic medications are also available to treat moderate to severe psoriasis with a safer side effect profile than the traditional oral medications used in years past. Biologic medications are either administered via subcutaneous injection by the patient at home or in our office depending on which medication is chosen. These treatment options are more convenient, more effective and safer for our psoriasis patients. Be proactive and set up your appointment with a dermatology provider to evaluate your psoriasis now so that you are prepared for the upcoming dry winter season.
Hair Loss We have been impressed with how often we have found a low level of iron "stores" (called Ferritin level) in women with a specific type of hair loss called Telogen Effluvium. This type of hair loss is usually more sudden than typical female pattern loss (which tends to be gradual but progressive) and occurs all over the scalp and not just on the top of the scalp (crown), like female pattern hair loss. Having a low Ferritin level is different than having low iron or anemia. One can have a low Ferritin level and still have a normal iron level. How does this happen? The human body has many documented feedback mechanisms that allow it to maintain a good balance of nutrients. As we know, women need more iron supplementation due to their menstrual cycle which results in a loss of blood cells and iron. Cells that have a lot of growth and turnover need iron (all cells need iron for their basic metabolic processes), and hair is one of those cell types that requires a lot of iron. I don't know this for sure, but I believe that when the storage tank (Ferritin level) gets low, the body might reduce some of the metabolic processes that it deems less important. In other words, with less iron in the tank the body may reserve the remaining iron for the most important processes, such as making new blood cells, blocking its use for something "less important" like regenerating hair. We check Ferritin levels when we see this type of hair loss and OFTEN find it is less than 80, which has been associated with hair loss. Iron replacement with over-the-counter iron available at the drugstore often does increase hair growth in this instance. It is important to be diagnosed with the proper type of hair loss as only a certain type of loss called telogen effluvium will benefit. Also, don't overdo the iron supplementation, as it can cause constipation.

PSORIASIS has made its mark on you. Here's a chance to make yours. Join our psoriasis registry. PSOLAR is looking for 8,000 volunteers to participate in an eight year registry to
observe and collect information from patients with moderate-to-severe psoriasis. No medications will be administered and you will not have to change your current psoriasis treatment or care.
You may qualify if you:

- Are at least 18 years of age
- Have been diagnosed
- Are willing to participate in follow-up with psoriasis visits every 6 months
