When you get this I will have just been to Nice, France for the 5-Continent Congress International Laser meeting. I love going to international meetings since I get to see many products and equipment that are not available in the US as it always gives me ideas or just a preview of what's coming. I will be presenting our work with the Matrix RF at this meeting. We have been working on a new protocol with "sublative rejuvenation" using the Matrix RF (previously announced in the last newsletter for wrinkles) to treat people with acne scarring. We are especially seeing good results in patients with darker skin types.We are also excited about the fact that Sculptra®Aesthetic just became approved for wider cosmetic applications. We are proud to be one of the exclusive Sculptra®Aesthetic injection centers as only dermatologists, plastic surgeons and facial surgeons have been authorized to utilize this filler.There is also a new botulinum toxin that has been approved and we have it! Dysport™ has finally arrived. How is it different from BOTOX® Cosmetic? It is more similar than different but there are some subtle distinctions. According to the studies it may have a quicker onset, it appears to last just as long and it has a slightly different "look". Let's face it, it's hard to improve on something that is already a home run (anybody out there who doesn't like their BOTOX®?), but we welcome the diversity and the competition. We appreciate the time each person took to complete and return our surveys. The answers were compiled by an outside consulting firm that we have been working with for the past six years to optimize our patient and client experience. We were so happy to find out that you approved of our practice with an overall 95.9% Patient Satisfaction. This was a higher percentage than the overall national database (consisting of the top 500 dermatology practices nationwide) and an increase from our previous year's results. In addition, overall 98% of you stated that you would recommend the practice to family and friends. Thank you, I cannot tell you how gratifying that is, as we have worked very hard to earn your trust and are on a continual improvement quest. We also took all comments in earnest and have and will continue to study the feedback we received to try to find ways to improve our customer service and outcomes in every way we can. We have changed our schedules and added new providers in order to see you in a more to keep you waiting in the office less.The number one complaint was wait time in the office. We are taking many internal steps to correct this concern but we also need your help in order to do so. In order to optimize our patient flow, it is necessary for everyone to come to their appointment and to arrive on time. When people cancel at the last minute, arrive late, or do not show for their appointment, it affects the daily flow of our schedule and leads to the providers running late. In addition, it does not allow us ample time to contact our patients that are on our wait list who would like to be seen sooner. Please see our new financial policy, available on our website, that states our new protocol for handling late cancellations, late arrivals and no shows. We appreciate your cooperation and respect for our efforts in improving our patient flow!In addition, I am thrilled to say that thanks to our we have more appointments available for our patients two new dermatologists that joined us this summer. Our new physicians Drs. Garretson and Wang are outstanding dermatologists and dermatologic surgeons and they complement our wonderful, talented and highly experienced physician assistants Shannon Morrow and Emily Oehler. Our team of providers are eminently capable of taking care of your needs medically, surgically and cosmetically. Of course, I would love to see every one of my patients but I am only one person, thus I have created a practice where I feel that every provider is outstanding and will give excellent care equal to my own. I consider Drs. Wang and Garretson as well as Emily and Shannon as extensions of myself and trust them implicitly and would send my own family members to them for any and all care. Facial Wrinkle Correction and Volume Replacement with Sculptra®Aesthetic Welcome to the new age of anti-age! Sculptra®Aesthetic is a cosmetic facial injectable that works gradually, resulting in a more natural youthful looking appearance that can last up to two years. Dysport™: A New Treatment for Wrinkle Reduction Dysport™ (abobotulinumtoxin A) is a simple, effective, non-surgical treatment that relaxes facial muscles, thereby smoothing away facial wrinkles, similar to the way BOTOX® Cosmetic works. Some wrinkles on the face occur from muscle movement and the passage of time. When you frown you flex your facial muscles creating lines between your eyebrows. Over time these lines become deeper and last longer. Dysport™ is performed by injecting a tiny needle of medication into the facial muscles that cause the wrinkles. Dysport™ works by relaxing these facial muscles. This improves the wrinkles that are already present and can prevent the wrinkles from worsening over time. Because the treatment takes less than five minutes to perform, most patients find this extremely tolerable. Typically the relaxed smoothing effect develops within 1-2 weeks after treatment and lasts 3-4 months. Dysport™ was approved by the U.S. Food and Drug Administration (FDA) in late April 2009 and has been used for years in other countries with great success treating a variety of medical conditions including facial wrinkles. How to Treat Sun Damage on Your Neck The best way to prevent poikiloderma from occurring in the first place or preventing it from getting worse overtime is to wear a broad-spectrum sun block daily and reapply every two hours when outdoors for prolonged periods of time. Look for sunscreens that contain zinc oxide and titanium dioxide with a sun protection factor of 30 or higher. These active ingredients are known to provide the widest range of protection against both UVA (aging) and UVB (burning) rays. Sun protection clothing and hats such as SUNSOUL or Coolibar are also a great way to guard your skin.
Several treatment options have been shown to be successful in reducing the appearance of poikiloderma. Skin care regimens that contain bleaching agents and vitamin A derivatives are helpful in reducing the brown discoloration of the skin. Chemical peels along with water based microdermabrasion (SonoPeel™) can also be utilized to reduce the excess melanin and improve the texture of the skin. These above modalities are limited however since they do not reduce the amount of telangiectasias (tiny dilated blood vessels).
The best treatment known to reduce all of the components of poikiloderma and improve the condition a great deal is Photorejuvenation. During each session an intense pulsed light device is used to reduce the number of telengiectasias, fade brown discoloration, and stimulate collagen growth to smooth skin texture. A series of five sessions is recommended on average but more treatments may be necessary depending on the severity of the condition. The treatments do not typically require any down-time and only slight discomfort similar to a rubber band snap is felt during the procedure. Treatments are most effective if done in succession spaced about a month apart. So start your treatments early in the Fall to reveal your new neck in spring. Laser for Psoriasis and Vitiligo Our patients have been very pleased with their results from the XTRAC Excimer Laser over the last year and a half! Many patients have achieved a greater than 75% improvement in their psoriasis and facial vitiligo has repigmented an average of 90% of color in our patients! We have implemented a new step down treatment for our psoriasis patients as well to achieve better, long lasting results for patients. Most insurance plans have been very good about covering the laser treatments as well. In order to undergo treatment, a medical visit is needed in our office to review what medications have been used in the past so we can do a prior authorization with insurance and determine that you are a good candidate for the treatment. Treating Mild to Severe Acne One of the things that Advanced Dermatology truly excels at is acne therapy. We have just about every conceivable treatment and angle you can take with acne, all the way from early teen acne to severe scarring or adult acne. In addition to the full range of topical therapies available in Skinfo® which include exfoliants, benzoyl peroxides and retinols, we lack nothing! These topical products are inexpensive too and don't require a prescription. We even have many non-oily moisturizers to keep you from getting too dry from your topicals. If your acne is too severe to be controlled by topical therapy we have oral antibiotics, oral hormonal therapies (if you are female) and/or isotretinoin (accutane). We are highly skilled isotretinoin providers and know how to screen you to make sure it is safe for you and how to shepherd you through the process so that you are comfortable. If oral medications are not your thing we have some alternative therapies, Isolaz Deep Pore Lazr Therapy and photodynamic therapy, both often effective and neither involving any pills; unfortunately these are not covered by insurance. We also have chemical peels, cortisone injections and even occasionally short or low dosages of cortisone that can get you ready for an important wedding, bar or bat mitzvah or other major event. Finally we even do clinical research trials at SKINQRI so that you might be able to experience a treatment before it is available to the public. Two major companies that make topical acne medications turned to us to help them perform their FDA approval trials.
Tanning Bed Warning from the World Health Organization In July, the International Agency for Research on Cancer (IARC), a working group of the World Health Organization, announced
that tanning beds and other ultraviolet radiating emitting devices were added to the "Group One" list of the most dangerous forms of cancer-causing radiation; those that are carcinogenic. They join the list alongside tobacco and arsenic. The research was published online in the August issue of the medical journal, The Lancet Oncology. The team of scientists from the IARC analyzed results from about 20 studies. Tanning beds were added to "Group One" based on consistent evidence of a link between UV radiation from tanning beds (which largely give off ultraviolet A rays) and all 3 forms of skin cancer, including melanomas. The studies also concluded that the risk of skin cancer jumps nearly 75 percent in those that use tanning beds before the age of 30. Previous studies showed that younger people who regularly use tanning beds are eight times more likely to get melanoma than people who have never used them. PLEASE do not agree to let your teenager go into a tanning booth! This is risky behavior, no different than offering to buy your teen a pack of cigarettes or drink alcoholic beverages!
Do You Suffer From Excessive Sweating? Hyperhidrosis or excessive sweating of the armpits, palms, soles, or face is a common dermatologic condition that has a tremendous impact on the
quality of life of affected individuals. Fortunately, there are many treatment options available to control this embarrassing condition. First, prescription deodorants which block sweat glands and decrease sweating are available. We have a new prescription deodorant available called Hydrosal® which is very effective in reducing sweating without the irritation associated with other prescription deodorants. Another treatment option for hyperhidrosis is an oral medication called glycopyrrolate. Glycopyrrolate is an anticholinergic drug, which works by blocking sweat glands from receiving the message to produce sweat. There is a risk for side effects with this oral medication but most patients don't experience any. Iontophoresis is also an option for treating hyperhidrosis. It is a simple device that uses water to conduct a mild electric current through the skin's surface. It works best on excessive sweating of the palms and soles. Lastly, BOTOX® is an excellent option for treating hyperhidrosis in the hands, palms and feet that lasts an average of 6 months per treatment, but does require preapproval from insurance companies in order to get coverage. Otherwise it can be provided for an out-of-pocket fee. Discuss these safe and effective treatment options for hyperhidrosis with one of our providers today.
Sun Damage on Your Neck Now that the colder season is just around the corner some of us are looking forward to wearing turtleneck sweaters again so we can cover up our blotchy necks. Like
many other aesthetically unappealing skin changes, this one is also induced by recurring sun exposure and it is more prevalent in fair-skinned individuals. The medical term for this benign but chronic condition is Poikilodorma of Civatte. Often the skin is reddish-brown on the sides of the neck and chest but much lighter and even in color right under the chin where it is shielded from the harmful effect of the sun. The redness is due to a network of tiny dilated blood vessels called telengiectasias, and the brown discoloration comes from excess melanin in the skin. In addition to the color changes, textural alterations can also occur. The skin often takes on a "chicken-skin" like appearance, may become thinner and wrinkled. Eczema (Atopic Dermatitis) Study
SKINQRI is currently conducting a case study for people with widespread, mild to moderate eczema.
