SUMMER July - September 2008
Adv Derm Header
Dear patients, friends and colleagues,

We are thrilled to announce that Dr. Elaine Kung will join our staff of providers as an associate physician this summer. Dr. Kung, a native of New York City, graduated from Yale University with a bachelor of science in biology, magna cum laude, and Phi Beta Kappa. She continued her medical education at Yale School of Medicine and completed her residency in dermatology at University of Chicago Medical Center. During her final year of residency, she served as Chief Resident and received numerous awards for her research in tissue engineering, including the Young Investigator Award from the American Society of Dermatologic Surgery and one of the Grand Jury prizes from the 21st World Congress of Dermatology. Due to her comprehensive training, she enjoys practicing pediatric, medical as well as procedural dermatology. She has always had an appreciation for aesthetics with her love for fashion and her participation in the Miss Hong Kong Beauty Pageant. Dr. Kung will be seeing patients on Mondays, Tuesdays and Thursdays from 10:30 AM to 7:30 PM beginning August 18th. She will also be here most Tuesday and Thursday afternoons in July.

On June 25th we had our annual Staff Education Day. As always, we are constantly educating ourselves on how to provide the optimal level of care and experience for our patients and clients. We were delighted to have guest speaker Tena Brown present to us "How to Have an Unforgettably Positive Office Visit." For over 20 years, Tena has been an unforgettable patient advocate, psoriasis consultant, motivational and inspirational speaker, and is also the only non-medical person to regularly deliver workshops at the American Academy of Dermatology National Conferences. We discussed technological advancements, clinical research studies, and new skincare products. We also reviewed both clinical and cosmetic dermatology conditions and treatment therapies.

It's summertime finally and that means we need to prepare, protect, and treat our skin properly. Refer to medical news for information on summer rashes, bug bites, warts, molluscum and leg veins. In aesthetic news learn about the newest technology for cellulite reduction, hair removal, and chemical peels. Also, don't forget about your sunscreen. See Skinfo's news to learn about advanced sunscreen technology that comes in micronized formulas, powder, and in fabric clothing too.

If you or your child refuses to put on sunscreen due to the fact that you don't like creams, you are in for a surprise when you try our waterproof powder and sun-protective clothing. We also are introducing the Silk'n™, the first home based doctor-prescribed laser hair removal device, and SUN:SOUL Therapeutics hats and shirts, a brand new line of clothes that therapeutically treats your skin while you are outside! And yes, these new technologies really do work! These have been featured on the Rachel Ray Show, Dr. Phil, the Today Show and more. See Skinfo news to see how you can benefit from these.

Finally, I am excited to announce that Skinfo has donated to the Arbor Day Foundation in honor of Earth Day and Arbor Day in April and to the Skin Cancer Foundation in honor of Skin Cancer Awareness Month in May. We thank all of our clients who contributed to the success of these donations and look forward to contributing again in the near future.
Dr. Taub
Best Regards,
Amy Forman Taub, MD
And the Staff of Advanced Dermatology, Skinfo and SKINQRI.

Leg Vein Treatments Summer is here which means bathing suit and short season is here too! If you find yourself choosing capris over shorts because of spider veins on your legs, it may be time to have those unsightly veins treated. With a combination of sclerotherapy and laser treatments you can improve the look of your legs and be more confident to show them off in any wardrobe you choose! Most individuals will need a series of 3-6 treatments that are typically done at 6-8 week intervals. The vessels appear bruised or may be darker after a treatment, but with a new topical product called Cellex-C Spider Vein Complex the legs can heal and look better quicker so you can get back to those shorts and bathing suits over the summer. If you would like to learn more about the leg vein treatments available at Advanced Dermatology, call to schedule a consultation so we can tell you if these treatments would be beneficial for you.

Laser Hair Removal Laser Hair Removal is a light based treatment used to permanently reduce hair. Currently we use many different lasers (Prowave and CoolGlide Vantage by Cutera and the Comet and Aurora by Syneron) for hair removal treatments which allow us to safely and effectively treat any type of skin and hair color. Unfortunately, with every hair removal treatment, maintenance is needed because the body naturally over time produces new hair cells. We encourage our patients to have maintenance treatments on a routine basis to treat new growth. For the facial area maintenance is generally recommended every 3-4 months. For the body, some patients only need 1-2 maintenance treatments a year, but can range from 1-4 treatments a year. Some patients find this difficult to commit to. Finally, there is a new at home hair removal system, available for purchase only through physicians, called Silk'n™ that allows patients to manage and enhance their long-term hair removal results on their body. The Silk'n™ is an innovative device that uses Home Pulsed Light™ technology to deliver a safe and effective level of light energy. It can be used in the privacy of your own home and is safe for use on the body, easy to use, portable and cost effective. Contact Advanced Dermatology to set up a consultation to see how you can benefit from the Silk'n™.

Cellulite by Thermage In March a new Thermage tip for the treatment of cellulite was FDA approved. The deep heating radiofrequency in the ThermaTip™ CL tip rebuilds collagen connective tissues and improves blood flow to the tissue helping to improve and smooth the skin's surface. Results can be seen after one treatment, improving over the 6 months following the treatment. Clinical studies have shown results lasting as long as 18 months following a single treatment. As with almost all cosmetic treatments, combining multiple therapies together usually leads to better and quicker results. A great treatment option is to combine a Cellulite by Thermage treatment with a series of VelaShape™ treatments and an at home regimen such as Mesoestetic Anti-Cellulite Gel, Reducer Gel with Caffeine, or Toning Gel, or Cellex-C Cellulite Smoothing Complex.

BetaPeels and SonoPeels  Patients often ask what they can do during the summer to reduce sun damage, treat acne, or maintain results from previous treatments. Most laser and light devices rely on a pigmented target (freckle, blood vessel, hair follicle, etc). If we treat over an area that is tan or recently exposed to the sun, the active melanocytes and extra pigment in the skin also act as a target for the laser energy. This increases the risk of complications or requires us to turn down the energy to a safe enough level which may not always be effective. A great alternative in the summer is SonoPeels™ and BetaPeels. The SonoPeel™ uses ultrasonic energy and gentle cavitational forces to remove the dead, scaly outer layer of the skin, called the epidermis. BetaPeels use salicylic acid, also known as beta-hydroxy acid, to gently exfoliate the dead surface cells and promote regeneration and renewal of the skin. Treatments will reduce sun damage, improve the skin's overall tone and texture, reduce acne breakouts and can safely be performed during the summer!

Summer Skin Concerns Summer is here and with the beautiful weather we will all be outdoors more often. With this comes all of the wonderful rashes/eruptions that cause us to itch like crazy! The most common problems include PMLE (aka sun poisoning), bug bites and poison ivy. Here are some tips to try to avoid these problems and to treat them when they do erupt:

PMLE (sun poisoning) is best avoided by using a physical block that contains zinc oxide and/or titanium dioxide. Regular over the counter sunscreens that claim to have UVA/UVB protection will not help to prevent this condition if these ingredients are not present, so read your labels! Wearing sun protective clothing (coolibar.com, also available in skinfo) is also helpful and if you do have a rash from the sun, an over the counter cortisone cream will often help to calm down the reaction. Prescription products can be necessary with a severe eruption.

Bug Bites Use insect repellent to help prevent the bites from occurring, but if you do get a bug bite take an antihistamine (Claritin, Benadryl) and apply ice and hydrocortisone cream to relieve the itching. If you are VERY prone to a more intense reaction to the bite, you may benefit from taking a daily antihistamine to help prevent the swelling and itching. Please refrain from applying sunscreens combined with insect repellant! We need to reapply sunscreen often and if insect repellents are applied too frequently they can cause toxicity in our nervous system.

Poison ivy or sumac If you come in contact with a "poisonous" plant be sure to take a shower right away to get the rhus antigen (the chemical that is the allergen) off of your skin. If you think you got it on your clothes be sure to wash these items as well and gives animals a bath if they have been rolling in the plant. Usually you need a higher strength topical steroid than OTC products to get rid of the rash, but antihistamines, cortisone creams and compresses like Domeboro's can help to soothe the skin.

Warts and Molluscum Warts are non-cancerous skin growths caused by a viral infection in the top layer of the skin. Viruses that cause warts are called human papillomavirus (HPV). Warts are usually skin-colored and feel rough to the touch, but they can be dark, flat and smooth. The appearance of a wart depends on where it is growing. Warts are passed from person to person, sometimes indirectly. The time from the first contact to the time the warts have grown large enough to be seen is often several months. We offer a wide variety of successful treatments for warts and we are able to customize treatments for each patient. The treatments include a prescription cream applied to the warts daily, an oral pill taken daily, freezing the wart with liquid nitrogen, or an injection into each wart with either an anti-cancer drug called bleomycin or an immunotherapy drug called candida. Molluscum contagiosum is also a common skin disease caused by a virus which affects the top layers of the skin. The name molluscum contagiosum implies that the virus develops growths that are easily spread by skin contact. Similar to warts, this virus belongs to the poxvirus family and enters the skin through small breaks of hair follicles. It can also be spread through a warm pool or bath water. Molluscum is usually small flesh-colored or pink dome-shaped growths that often become red or inflamed. They may appear shiny and have a small indentation in the center. We have effective treatments to eradicate molluscum. These treatments include cantharidin also known as "beetle juice" which is painted on each molluscum, a prescription topical cream used at home, or removal of the molluscum via liquid nitrogen or curettage.

Leg Veins: Reticular vs Spider vs Varicose Veins Now that we have shorts weather here we might notice a few more unwanted spider veins on our legs. Most veins on the legs are there due to superficial stretched small reddish, purple veins called spider veins (less than 0.3 cm - about 1/5th inch) or larger blue reticular veins, 0.3-6 mm vessels (up to 1/5th inch). These vessels are not necessary for proper functioning of the leg and thus are a cosmetic annoyance. These can usually be treated with excellent results with sclerotherapy and/or laser treatments in our office. During a consultation, the provider will assess your veins to determine which treatment plan will benefit you the most. In many cases a combination of sclerotherapy and laser treatments yield the best result. (See Aesthetic News for more info on these treatments.) Cellex-C Spider Vein Complex, available at Skinfo, is an easily applied spray containing ingredients which stabilize blood vessels, making them less stretched and leaky and appear lighter. It improves the appearance of leg veins alone and is also a great adjunct to sclerotherapy and laser to speed healing between treatments. Some leg veins are just too large or advanced to be able to benefit from the procedures we offer at Advanced Dermatology. These are very large vessels called varicose veins, which are greater than 0.6 cm and usually blue, ropy and elevated off the surface of the skin. These veins frequently are associated with an underlying abnormal valve behind the knee or in the groin which needs to be fixed in order to have successful treatment of the varicose vein. This valvular treatment usually requires either a more invasive laser procedure, ultrasound-guided sclerotherapy or surgical removal. Don't hesitate to make a consultation visit. We will let you know what the appropriate therapy is for your legs and if we cannot help you we will direct you to another respected professional who can perform these more invasive procedures.

Silk'n™ by Home Skinovations* Silk'n™ is a personal Photo-Epilation device for long-term hair removal. The term photo-epilation describes the use of pulsed light to remove unwanted hair. Silk'n™ removes hair by emitting a pulse of light that is absorbed by pigment in the hair shaft beneath the skin surface. This disables the hair follicle and delays hair growth. Silk'n™ has been designed for safe, convenient and effective hair removal in the privacy of ones own home. It is recommended for treatment of brown or black pigmented hairs on the underarms, bikini line, arms and legs (not designed for facial hair removal). This device is only available through physicians due to its highly sophisticated technology. Although we do retail Silk'n™ in Skinfo, each client must consult with Advanced Dermatology to discuss how the Silk'n™ will benefit them prior to purchase. Finally everyone is able to "Experience the Silk'n™ Revolution." **We apologize for our error in the printed newsletter. The name is Silk'n™, not Silk'k™.
