Happy New Year!

 

Hope you have had a wonderful holiday season. I traveled to Buenos Aires for the World Congress of Dermatology in October. It was a great trip! The people are friendly, the food is great (they specialize in steak as there are many ranches all over this very large country), we watched a number of different tango styles (none of which I would attempt!) and we schooled ourselves in the history of this very European City. My presentations went well and the meeting itself was truly stimulating; international meetings always give me a unique perspective with different ideas for treatment options and ways to improve our care.

 

In this issue we wanted to let you know about some of the issues and new information regarding Vitamin D supplementation, the factors we use in deciding what drugs we use for moderate to severe psoriasis, and the advances we continue to make in cosmetic dermatology. Combination therapies (combining more that one treatment option) are touted by most cosmetic dermatologists as being the way to get the most dramatic results. I wrote an article on this that was recently published in Skin and Aging News, so I will share some of my thoughts with you.

 

We are constantly attempting to improve our level of service. We have hired a third cosmetic coordinator whose job will be to make your stay at Advanced Dermatology while you are undergoing any treatments as pleasant and comfortable as possible. We hope to have some additional staff in the near future to facilitate your entry as well as answer the phones so we can provide you with the best service possible.

Combining Tissue Tightening techniques for optimal improvement.  We have multiple Tissue Tightening devices: Thermage, ReFirme and Titan. All have had many exposures on national TV shows, including Oprah, Today, the View, and the Rachel Ray show. Which one is the best?, people often ask us. We think they are all good, especially with appropriate technique and an esthetic training to know how to get the best results. We have found that for thick and/or oily skin we like Thermage, which penetrates the deepest of the three. For medium thickness skin we like ReFirme and for thin skin we choose Titan. However, most people have a combination of all three so we have started to combine them in an effort to get best results. By combining 2 or even all 3 of these, we can affect all levels of the skin to respond to the call for firming and creating new collagen. We are probably one of only a few centers that have all 3. If it seems confusing sometimes in your consultations because we have so many choices, just remember that we have so many because we feel we can truly individualize your care best that way. Sometimes one laser works great on one person whereas another works better on a different person.
 
Combining Tissue Tightening with the Velashape, especially on the abdomen or area above the knee where fat and loose skin can both exist, can achieve better results than either alone. Combining tightening and fillers, BOTOX® Cosmetic and fillers, and Fraxel® SR1500 or Pearl and tightening are other ways to achieve great results. Most people don't only have one factor causing an aging appearance. By combining treatments we can provide you the best possible results.
 
Fraxel® SR1500 and Stretch Marks 
Although we have only recently begun to treat stretch marks using Fraxel® SR1500, the early results have been exciting. A series of treatments is needed, but results have been seen after the first and continue to improve with each treatment.
 
Hair Removal, Leg Veins Reduction, Cellulite Reduction, and Body Contouring  Now is the time to start preparing for your spring break trip! Get rid of those ingrown hairs in your bikini line with laser hair removal treatments using Prowave 770™, the newest of our hair removal lasers. Eliminate spider veins with laser leg vein treatments or sclerotherapy. Results with Perlane  Perlane is one of the newer fillers in our armamentarium. It is made of the exact same material and by the same company as Restylane, but it is a denser mix of particles. Its use is for volume enhancement, under the eyes, in deep nasolabial folds, marionette lines, cheeks, jaw line and lips. Perlane is robust- it gives us much more volume for the same amount of material than either Restylane or Juvederm. Most of the time, we use Perlane in combination with Restylane or Juvederm, to get the right mix for volume and lines. As our experience with fillers has evolved, we are finding that full volume correction at your initial visit is ideal. This means that we will use the most material at your first visit, followed by periodic touch-ups every 6-12 months as maintenance. Instead of using a small amount of filler and needing to refill everything after it reabsorbs, we can "top it off" periodically which reduces your cost, keeps your results in best condition all year long and decreases the incidence of bruising with your maintenance treatments. One of our favorite procedures, Shannon, Emily and I truly enjoy and work hard at giving you the most natural looking filler enhancements. This is both exacting science and artistic creation and we are proud of our skills.

Vitamin D and the Sun    UV rays from the sun trigger Vitamin D synthesis in the skin. Some researchers have stated that we should avoid sunscreen in order to get more Vitamin D in our "diet". An increasing body of research supports the hypothesis that the active form of vitamin D has significant, protective effects against the development of cancer. Many internists are now telling their patients to get unprotected sun exposure daily to make sure that they get enough vitamin D. The problem with this recommendation is sun exposure leads to skin cancer and melanoma (which can lead to death if not diagnosed early), not to mention sun freckles, wrinkles and sagging of the skin. The truth is, no one applies enough sunscreen to adequately protect themselves from the harmful UV rays. We do not apply 15-30 min prior to exposure, we forget to re-apply every two hours and rarely does someone use an ounce of sunscreen (the recommended amount) on their body (this is equivalent to a shot glass size).
 
Currently, most experts in the field believe that daily intake of Vitamin D supplements between 1000 and 4000 IU will lead to a more healthy level of Vitamin D in the bloodstream which will offer protective effects against cancers of the breast, colon, prostate, ovary, lungs, and pancreas (Ingraham BA, Bragdon B, Nohe A. Molecular basis of the potential of vitamin D to prevent cancer. Curr Med Res Opin. 2007 Nov 21). Based on the available evidence, we recommend that all adults take a minimum of 1000 IU of Vitamin D per day and minimize unprotected sun exposure. Please consider continuing to use your sunscreen as much as possible and increase your vitamin D levels through supplementation.
 
Update on Psoriasis Biologics
  Psoriasis is a chronic skin disease in which scaly, inflamed patches appear on the skin that may itch or even be painful. Treating psoriasis is often frustrating because conventional therapies are messy and time-consuming without delivering good results. Fortunately, new therapies have been approved over the past few years that are more effective and easier than ever. These medicines, called biologics, are derived from proteins and work by blocking interactions between immune cells. Biologics are both safe and effective in clearing up psoriasis and maintaining clear skin for years, although as with all medications, there are potential side effects which your providers will discuss with you, should you consider these as a treatment option. Since biologics are administered through injection or IV infusion, most patients find them easier to use than conventional therapies, which often require large surface applications of creams or ointments. Combination therapies of biologics and systemic medicines, light treatments, or topical creams have shown increased efficacy. There is more good news on the horizon. Researchers are making strides toward identifying the exact causes of psoriasis and new drugs are currently being studied to treat this challenging skin disease.

Keratosis Pilaris If you have tiny white or slightly red bumps on your upper arms or thighs, you may be among the 40% of the population that has a condition known as keratosis pilaris. It is so common, that if you do not have it, chances are that someone you know does! Keratosis pilaris, referred to as KP for short is a benign condition in which keratin, a protein in the skin, forms hard plugs within the hair follicles. KP may appear similar to "goose bumps" and often has a "sandpaper" feel. It runs in families and is more common in children and adolescents. Keratosis Pilaris usually worsens in winter months due to lower humidity. Again, although harmless, some may find it cosmetically unappealing. Treatment is geared at mild exfoliation and added moisturization which can diminish the bumps but is not a cure. Skinfo carries a host of products aimed at such treatment. These include: Mavena Lipolotion or Special Care Creme, Glytone Exfoliating Body Wash and Rejuvenate Facial Cream Step 2 or 3, and Skinfo Soft and Smooth Body Lotion. These products may be used in combination with Skinfo Soothing Cream for best effect if much redness is present. A Clarisonic brush also helps to exfoliate the dry bumps gently as well as increase the penetration of these ingredients. Our Skinfo Skincare Specialist or your provider will help you choose.  A prescription strength topical may also be recommended by your provider.
 
Research Header
Psoriasis Study  You may qualify for this study if you have been diagnosed with chronic moderate to severe plaque psoriasis by a medical doctor and are currently being treated with Raptiva or another biologic psoriasis treatment. Qualified participants will receive study-related medical evaluations at no cost. No medications will be provided as part of the study - all participants will continue with their current drug therapy. Reimbursement for your participation will be provided.

Eczema Study  This study is enrolling patients ages 2-17 years old with atopic dermatitis (eczema) who have used Elidel for at least 6 weeks in the past 6 months (they do not need to currently be using Elidel). Participants will complete a short survey which will be mailed to your home every 6 months for the duration of the study. No office visits are required. Reimbursement for your participation will be provided for every completed survey.

Eczema Study  Do you have eczema or atopic dermatitis? Have you used Protopic (tacrolimus) Ointment before you were 16 years of age to treat your eczema? Would you be interested in participating in a long-term program that will monitor your health? You may be eligible to participate in an observational research study. Please call us if you would like to learn more about this important program.

Skinfo® Product Study We are comparing Skinfo products to prescription and over the counter products.  Subjects will be given two products, one Skinfo product and a comparable prescription or over the counter product, and will be asked to rate the two.

Arnica Study Patients having filler to their undereyes and/or marionette lines will be given a kit of an oral and topical medications to reduce the amount and duration of bruising from the treatment. Subjects will be asked to complete a questionnaire and photographs will be taken.

Restylane/Perlane Study  We are looking for 10 subjects between the age of 40-50 with moderate aging of the face, especially under the eyes, the cheeks, nasolabial folds, and marionette lines.  Subjects will receive one treatment of 4-6 syringes of Restylane and Perlane and will be followed for the 9 months following the treatment. Subjects will receive 50% off the usual fee for this treatment. Interested participants must be willing to have photographs and release them to Advanced Dermatologyand SkinQRI. A cosmetic consultation needs to be performed prior to enrollment in this study. Bruising Study Patients having injectable filler treatments will be randomly assigned to one of four groups, including two topical and oral medications and two in office procedures, to reduce the amount and duration of bruising from the treatment. Subjects will be asked to follow up one day and one week following their injectable filler treatment, complete a questionnaire, and photographs will be taken and released to Advanced Dermatology and SkinQRI.  After completion of the one week visit, subjects will be given a $250 stipend.
