Mini Grants to Address Chronic Disease and Health Disparities

CHNA logoh Coalition of MetroWest (CHNA 7) is accepting applications for its 2012-2013 Mini-Grant Cycle. Approximately $30,000 is available to organizations with projects that address chronic disease and health disparities in a 22-town region.

The mini grants are earmarked for projects focused on chronic disease prevention or management for disadvantaged, low income and cultural, ethnic and linguistic populations.

Mini grants of up to $3,500 will be available for projects taking place between J
uly 1,2012 and June 30, 2013. Proposals are due on May 21 and funding decisions will be announced in June.

"The mini-grant program aims to enable local groups and organizations to take on small but significant projects to improve health and wellness in the area," according to a statement from the CHNA.

Living with Diabetes  
 

Maureen PorcellaBy the time Maureen Porcella, 70, attended a diabetes education class earlier this year she had already been living with diabetes for 20 years. As far as she was concerned, she knew everything she needed to about how to manage her chronic disease.

 

But, to Porcella's surprise, she learned something from the class which was offered as part of a program called "Managing Diabetes in Seniors: A Community/Medical Partnership." The program is funded through a grant from the Foundation under its Healthy Aging Initiative. It is a partnership between Jewish Family Service of Metrowest in Framingham and Southboro Medical Group in Southborough.

 

"By the end of it I thought, 'Whoa, this was really interesting,'" said Porcella who lives in Ashland.

 

The program was designed to reach people like Porcella - older adults who need help (whether they realize it or not) managing their diabetes. The goal is to improve their glucose levels through exercise and diet and reduce hospitalizations.

Violence Prevention desk calendar
The Framingham Board of Selectmen and the Violence Prevention Roundtable are hosting a community forum about violence prevention at 7 p.m. on Monday, April 23.

Medication Error Forum 
The Natick Service Council is hosting an information session called "Medication Mistakes and Misuse" at 6:30 p.m. on Thursday, April 26. The free event, which is part of The Living Well Project, will be held at the Morse Institute Library at 14 East Central St. in Natick. For more information, call 508-655-1791.

Teen Pregnancy Prevention Coalition
The Framingham Teen Pregnancy Prevention Coalition is hosting a community breakfast called "Building a Healthier Community for Teens" featuring a keynote address from Sophie Godley of the Boston University School of Public Health. The event will be held at 7:30 a.m. on May 31.

Barr

 

Henry Barr, chairman of the Foundation's Board of Trustees was honored by his alma mater this week when he received the 2012 Distinguished Alumni Service Award from the University of Massachusetts at Amherst.

Barr, who is a senior partner of the Law Offices of Barr and Cole in Newton and a member of the Class of '68, was honored along with five other alumni during a luncheon held April 17 at the Massachusetts State House.
