For a match to be determined you must have applied for your NPI using specific information. The following outline is how your NPI information should be structured.
Type I Individual NPI -
Should list the individual provider's:
- Name
- Social Security Number
- Medicare PIN
- Medicare UPIN
- Medicare NSC for materials, if applicable*
*List only if you are NOT a group
Type II Organization/Group NPI -
Should list the group's:
Special Note: If you are enrolled in Medicare as a group, you must have two types of NPIs: one for the group and one for each individual provider within the group. If you do not have a group NPI, it is not too late to apply for one. Please DO NOT WAIT! |
