Greetings!

Medicare states, "Beginning September 17th, 2007 Medicare will begin editing the NPI/Legacy ID combinations for validity against the NPI crosswalk file. Where a match cannot be located on the crosswalk, claims will be rejected or returned to the provider."
 

For a match to be determined you must have applied for your NPI using specific information.  The following outline is how your NPI information should be structured.

Type I Individual NPI -
Should list the individual provider's:
  • Name
  • Social Security Number
  • Medicare PIN
  • Medicare UPIN
  • Medicare NSC for materials, if applicable*

*List only if you are NOT a group

 
Type II Organization/Group NPI -
Should list the group's:
  • Business Name
  • Tax ID
  • Medicare PIN
  • Medicare NSC for materials, if applicable

Special Note: If you are enrolled in Medicare as a group, you must have two types of NPIs: one for the group and one for each individual provider within the group. If you do not have a group NPI, it is not too late to apply for one. Please DO NOT WAIT!

Please review your NPI information to ensure that it has been set up correctly. If you find that you need an NPI and/or your NPI information is not correct, you must apply and/or make all necessary corrections by the September 17th deadline. Otherwise, your claims will be denied.
