Although this month's bulletin addresses a hazard in medical technology, and many of our audience are in other industries, largely oil and gas, the lessons learned still apply. They are the importance of careful design and construction, safety procedures, and training.

When someone receives a cancer diagnosis, they worry about their prognosis and what treatments will be prescribed, but never the integrity of the treatment machines. In the 1980s, a computer program accidentally delivered massive doses of radiation to patients, creating the worst medical incident in the history of this type of machine.
