Practical Lessons From Major Accidents
Vol. 3, No. 1 |
Greetings!
Heads up! Safety cases appear to be one step closer for drilling in deepwater Gulf of Mexico (GOM). Tuesday January 11 at the National Press Club, the National Oil Spill Commission released its final Report to the President, Deep Water - The Gulf Oil Disaster and Future of Offshore Drilling. (See http://www.oilspillcommission.gov/.) Again, requiring safety cases is a strong recommendation of the Commission. Some companies already create them as a matter their performance-based HSE culture. Other GOM thought leaders are not delaying; they are moving ahead with safety cases now to be prepared for the change later. The Commission recommends that Congress and the Department of the Interior collaborate with the International Regulators' Forum and other organizations to develop standards for the Gulf of Mexico, the Arctic, and "globally wherever the international industry operates." See ERM's white paper on this topic. Also, please take a just minute to tell us how we're doing with our bulletins and seminars. Complete the short survey.

Practical Lessons Bulletin
What happens when a 20-year-old, 20,000-barrel, 25-foot high, 195-feet circumference brewing vat pops a metal band? A veritable tsunami of beer bursts its vat, mows down buildings, fills basements, and, most unfortunately, kills. The story of the Great Beer Flood of 1814 London bears lessons applicable to today's industry.
