2012 Prescription Drug Formulary

Effective, January 1, 2012, Coventry Health Care will be making changes to our formulary. The list of formulary changes is available on our website. The Prescription Drug List (PDL) can also be accessed under the Formulary tab and includes Tier 3 (Large Group 51+) and Tier 4 (Large Group 51+ and Small Group 2-50) coverage.
All prescription documents are located on our website and can be accessed using the following breadcrumb:
