In an effort to better serve you, the Carelink Team has developed the attached Request for Quote Form. Effective immediately, this form should be submitted along with Statements of Health for groups requesting an October effective date onward.
The second page of the form asks for either a current carrier billing statement OR quarterly wage report. Obtaining this information at the time of the initial request will help us ensure the group is underwritten properly. This form will allow us to better strategize with you on new group opportunities. Coventry Health Care is pleased to introduce the NEW benefitexpress Version 2.0 (V2) with the following enhancements:
- updated user interface with a NEW LOOK and FEEL
- new proposals and renewal exhibits
- enhanced flexibility
- improved performance and reliability
The NEW benefitexpress V2 system will be available for new sales and renewals as of 12/01/09 effective dates.
To access the NEW benefit express V2, log onto https://be.cvty.com. The NEW logon for benefit express V2 will be available on September 2, 2009.
Please note that you will continue to utilize the current benefitexpress for September 2009 through November 2009 effective dates for new business and renewals. All other business with 12/1/09 effective dates and forward will be processed on benefitexpress Version 2.0.
We will be offering in-depth training sessions on our NEW benefitexpress V2 system for you and any of your staff members in the coming weeks. We will send out invitations to these trainings once the dates have been confirmed.

| New "Select PPO" Plans Available

Effective October 1, 2009, Carelink will offer three new Select PPO plans. These new plans will feature expanded deductible options of: $1000, $2000 and $5000. Select PPO plans which include deductible options of $500, $1500 and $2500 will continue to be available, but will have slight changes made within the existing benefit plan design.
