| EASY LOGIN FOR BROKERS

Brokers can now use a single login ID and password to access multiple employer groups who permit them access to Online Account Management. This service allows brokers to perform online functions granted to them by their employer groups without the need for multiple login IDs and passwords.
Brokers who wish to have this functionality should fax the completed form below to our CARE Team at 866-266-2206.

| COVENTRY DENTAL EXPANDS NETWORK WITH 4MOST

Effective July 1, 2009, Coventry Dental has partnered with the 4Most Dental Network expanding dental access throughout West Virginia. All Coventry Dental plan options will now include the 4Most Dental Network in addition to the Dentemax National Network.
"4MOST has quickly grown to become the largest independent stand-alone dental network servicing all of West Virginia and is positive the network will prove to be a cost saver to the consumer and a mechanism for the provider to grow their patient base," said Gary Shorter, VP Business Development for 4Most.
"Additional value added benefits of the network include a quality and credentialing check of all participating providers and patient savings by way of "no balance billing" of 4MOST discounts."

| IMPROVED GROUP APPLICATION PROCESS
|
We have made recent improvements to our group implementation process. Effective June 1, 2009 for new groups and July 1, 2009 for renewals, the Group Application should be completed and submitted by the group upon the sale or renewal acceptance. This simplified form will replace the Benefitexpress Employer Application and eliminate the need for group signature after implementation.
