A few recent accomplishments worth mentioning...

Eric competed in the Road to the Pogue beating his time from 2010

David and Eric both ran in Skip's Run on Father's Day.  Kudos to Dave for pushing his twins for all 4 miles.

Lois competed in her first swim meet and placed first in the 100 & 200 Freestyle in her age group.
Laurie Albert, coach of Green Mtn. Aquatics and Master's Team @ CCBA helped in getting her to this level.

Lastly, many of our Tuck Patients competed in the NJ Devilman
Triathalon on May 7th.  Congrats to all of you for your determination to battle through injury and FINISH!

And to the countless other patients that have been able to get back to running, back on their bikes, swimming and happily putting the miles behind them...Our hats off to you as well!

"It was a great weekend for Dartmouth Rugby, and to win a national championship to cap off my four years with the club was unbelievable.

Thank you for all of your help, my hamstring felt great all weekend!"

BE Fit had the opportunity to work with many Dartmouth athletes in their Spring 2011 seasons. Congratulations to all of you, and we look forward to working with some of you again in the fall.

Exercising in hot weather

We do not get a chance to experience real heat very often in New Hampshire, but for this very reason, we are not acclimated to it, and may not be aware of symptoms that could indicate a problem.

Heat stress to the body is not always caused by air temperature alone.  High humidity limits sweat evaporation, which is the most important avenue of heat loss for our body. Heat illnesses are most likely to occur in hot, humid weather, but can also be intensified with wind and thermal radiation (direct sun exposure) in the environment.

Common symptoms of overexertion in the heat are:

1- Muscle cramps - acute, painful, involuntary muscle contraction present during or right after exercise session.
2- Dizziness with standing up quickly, sometimes occurs right after cessation of exercise.
3- Inability to continue exercise due to heavy sweating and energy depletion.

More serious symptoms include pallor (white coloring in the face), weakness, swelling of hands and feet, and increasing core temperature.  If this progresses, it can cause changes in mental status such as disorientation, and vomiting.

Recommendations for Preventing Heat Illness-

- Modify activity under high-risk conditions, and consider rescheduling to avoid the hottest time of the day.
- Avoid radiant heat from direct sunlight as able, and conduct warm-up and stretching session in the shade or indoors.
- Progressively increase intensity and duration of work in the heat over a period of days or weeks because the body needs a period to adjust and accommodate.
- Wear loose-fitting, light colored, breathable and absorbent clothing to allow effective cooling.
- Fluid replacement with water, sports drinks, or sodium-containing fluids.  An easy assessment or goal is to keep urine a clear to light yellow color.
- If exercising intensely in these conditions, you should weigh yourself before and after exercise to estimate the amount of body water lost during exercise. Ensure a return to pre-exercise weight before the next exercise session. Consume approximately 16-20 oz of fluid for each pound of body weight lost during exercise.

"Hey Mom, my back hurts!"

In the United States, low back pain remains one of the leading reasons why people see their physician.  As many as 85% of all individuals will experience an episode of LBP in their lifetime, generally lasting 2-4 weeks.

With something so common, sometimes it is hard to determine when it is serious, and warrants medical attention.  If the pain stays local to the spine, is better with activity, but worse after slouching and/or bending forward for long periods of time, this falls into a typical LBP presentation.  It will likely improve with greater postural and positional attention, relative rest (not bed rest) and resolve on its own within 2 weeks.

Things that should raise your level of concern and suggest visiting your physical therapist or primary care physician include:

- Pain that is present constantly but varies in intensity, causes them to walk carefully and avoid activity, and worsens with activity.

- Low back pain with accompanying pain into the leg (Note that this is NOT an emergency, even if the pain is pretty intense, but seeing a physical therapist or doctor soon is advised).

- With low back pain after a fall, or a strong external force occurred, then the possibility of diagnostic studies being important is greater.  If it is not during regular office hours, then being seen in an Emergency Department may be helpful.  This is also true for pain accompanied by true numbness or significant weakness (not just pain) in one or both legs.

If you feel that any type of LBP is compromising your child's quality of life for more than 2 weeks, then they will likely benefit from an assessment. Your child may find that an assessment with a professional, and information supplied by this person, is better received than advice from a family member (just like for most of us adults!).

Remember, you know your child the best, and therefore you know if their complaint seems like other minor aches in the past, or is more severe or persistent.  Trust your instincts!
