Connections for Healthy Aging Launched  

Staying healthy and engaged as we age is no easy matter these days. But with a little help from our friends, we can be informed and empowered to do just that.   

 

The first introductory session launched two weeks ago and generated a "full house" for the four part class starting Marcia Peterzell talking in August.  

 

The next introductory session is set for: 

Friday, September 21

10:00 to 1:00 (including lunch)

Western Park Apartments, 1280 Laguna Street   

 

The four-part workshop series will follow starting  October 19th at the same time and  location.    

 

Workshop topics, presented in a fun and interactive way,  include:  

  • Assessing your own personal goals related to Healthy Aging
  • Learning how to build a network of support to help you reach those goals.
  • Important documents every senior should have prepared and stored in a manner that is safe, security and accessible in an emergency.
  • Proven ways to prevent isolation and loneliness
  • Tips and resources for how you or a loved one can survive a hospital stay and enjoy a smooth transition back home.
  • Simple, on-line tools that can support our efforts to be informed and empowered as we age.
  • Increase access to individuals and information that can strengthen our networks of support.

Planning for Long Term Care Services and Supports in San Francisco  
With so many changes happening in the world of health and long term care services and support, IT'S hard to keep up.  This is especially true foR seniors and persons with disabilities who have both Medicare and Medi-Cal (the dual-eligibles or "duals" for short).  In San Francisco, the community planning process for the new system is called the Long Term Care Integration project (LTCI).

 

The goals of the Long Term Care Integration project are to: 

  1. Emphasize individual choice and promote services that are directed by those who need assistance
  2. Create a system that is easier for people to use
  3. Promote the highest quality of care
  4. Assist seniors and adults with disabilities to remain in their community as long as possible, and
  5. Generate cost savings to build a better San Francisco health care system. 

Planning for the new system is being guided by agencies that provide health care to seniors and adults with disabilities. The lead agencies are the San Francisco Long Term Care Coordinating Council (LTCCC), the Department of Aging and Adult Services (DAAS), the Department of Public Health (DPH) and the city's two MediCal health plans, the San Francisco Health Plan and Anthem Blue Cross.

 

The planners are led by a 14-member Design Group supported by 50 additional members who work on three subcommittees - Finance, Communications, and Scope and Delivery of Services. They will develop a strategic plan that will guide the improved organization, availability and financing of long term care services and supports in San Francisco.

Meet Community Connector - Lenny Reiter

Lenny Reiter Lenny was one of the first people at the door ready to help when we started Community Living Campaign.  He has a long resume of efforts to make San Francisco a better place for those who might get left out or left behind.  The challenges he has faced in his own life have made him gracious and empathetic friend with others.  

I first met Lenny at Planning for Elders, where he and Constance Smith helped establish and coordinate a SeniorNet computer learning site at Dorothy Day community.  Lenny help teach classes, recruit new teachers and in general make sure people felt welcome as a part of PECC's Digital Bridge program.  These skills and experiences are ones he has brought to CLC as we help San Francisco renews its efforts to bridge the digital divid

e for seniors and adults with disabilities.  Lenny's background in interior and exterior design have been put to use in helping organizations like Planning for Elders better design their working place.  Prior to that, his design work gave him the opportunity to travel the world, living and working in Honolulu, Haifa, Rome and Venice, as well as various cities around the United States.  

Lenny is a great connector - he links CLC to the myriad of organizations where he volunteers or serves as a leader - Planning for Elders, Healthcare Action Team (HAT), Senior Action Network,  Creativity Explored, CPMC,

Kaiser, Open Hand and more.   He shares information and resources between these organizations and with CLC's other Community Connectors.  

 

You might recognize Lenny as the man behind the table to help distribute translation equipment (like he did at the various City Hall summits CLC was involved with) or see him in action as a member of the Health Care Action Team  discharge planning skit.  

 

We apprecate Lenny's cheerful energy and content readiness to lend a helping hand wherever he is.

New Basic Training Class!  - Monday, August 13 and 20, 1:00 to 2:30 p.m. 

At last, a 2-Part class for beginners on how to use the computer. The first day will include scan logo basic computer terminology and how to use a computer mouse. The second part will focus on basic Internet terminology, like what is a browser, what is a search engine, and how to locate information by using websites. If participants are ready, we can assist in getting email accounts set-up. This class is being hosted by SCAN Health Plan.  Please tell a friend who you have been trying to get started on the computer.

Linda Post has lived in San Francisco for over 40 years and is a long-time political activist, past president of the S.F. National Women's Political Caucus, and Chair of the San Francisco Democratic Party.
