Instead, the model we need calls for integrating primary care with mental and behavioral health, specialty care, HIV/AIDS, hospital and long term care services, and community resources. Going forward, responsibility for care will increasingly move toward high-performing, integrated health networks that include the entire spectrum of services -- and most importantly, is built on a foundation of excellent, high-performing primary care.
We are eager to hear from you as you think about and act on these challenges, and hear how PCDC's services might be shaped to help you and other providers achieve these goals. News from Washington - Primary Care Initiative Offers Bonuses for Coordinated, Cost-Effective Primary Care
|
 The U.S. Department of Health and Human Services (HHS) has launched the Comprehensive Primary Care Initiative (CPC). Led by the CMS Innovation Center, CPC will bring multiple providers, including Medicaid, together with Medicare to provide incentives and shared savings for primary care providers that improve care management and coordination. The effort is intended to create a critical mass of payers supporting primary care providers who cost-effectively manage care for high need patients, ensure 24/7 access to care, emphasize preventive care, engage patients and caregivers, and coordinate care across the "medical neighborhood." The initiative is voluntary and will begin as a demonstration project in five to seven health care markets across the country. Letters of intent are due November 15, 2011.

New Study Reveals Hypertension Risk Factors for Low-Income Populations

Although racial disparities in the awareness, treatment and control of hypertension have been well documented, few studies have focused strategies for addressing disparities among community health center settings.
In a recent article published in the American Journal of Hypertension, and co-authored by PCDC, Open Door Family Medical Centers, and Columbia/NYU, revealed that rates of hypertension among African Americans was more than double that of whites and Latinos, while the group was less likely to have controlled blood pressure than their peers. Authors identified several predictors of uncontrolled blood pressure including high body mass index (BMI), diabetes diagnosis, and lack of regular care.
Overall, the team found that low-income minority and immigrant populations with access to care in the four Open Door Medical Center sites studied had blood pressure control nearing or at the U.S. government's Healthy People 2010 goal of 50%. The study was funded by the Agency for Healthcare Research and Quality (AHRQ) to evaluate the effect of clinical decision support and an EMR-Based registry in improving hypertension management in a health center setting.

PCDC Financings: Expanding Access to Primary Care Capital Across New York State

New health facility for at-risk South Bronx Residents: With the help of a $357,000 PCDC loan, HELP/PSI is building the CitiWide Harm Reduction Health and Wellness Center, which will give hundreds of South Bronx residents coping with homelessness, HIV/AIDS, substance abuse and behavioral health issues a new place to go for primary, preventive and behavioral health services. Staten Islanders gain access to primary, dental care: A $1.5 million PCDC bridge loan is helping speed development of expanded primary care and dental services for the Community Health Center of Richmond, which is embarking on a $5 million New York State-funded project to construct three exam rooms and six dental operatories, in addition to administration and conference spaces. Bridge loans can facilitate project development when organizations are required to expend resources up front in order to draw down grant funds. Creating Jobs in Low- Income Communities; PCDC Wins National Award
|
Recognizing the importance of community lenders in spurring economic development, Starbucks and the Opportunity Finance Network have launched the Create Jobs for USA program. Intended to raise tens of millions of dollars through customer donations, 100% of funds raised will support Community Development Financial Institutions (CDFIs) in their efforts to finance community businesses and create and sustain jobs in underserved communities.
PCDC is one of just 28 CDFIs nationally to receive an award, which will be used to further our investment in community health facilities. Since 1993, PCDC has created an investment of $335 million in primary care projects and created or preserved more than 3,600 jobs throughout New York State.
