These are indeed challenging times filled with economic and political uncertainty.

 

If we truly want to save health care dollars, we need to continue investing in the areas that show the greatest cost saving promise. These include community health centers, patient-centered medical homes, coordinated care models, and community-based health initiatives. The faster we move on health system reform, the sooner we will realize cost savings and quality gains.

From rapidly expanding medical homes and "health homes," to developing a statewide health IT network, to actively restructuring the healthcare delivery system, New York and other states are moving toward an integrated system that values quality of care and saves patients, businesses and taxpayers billions of dollars. Partnership with and resources from the federal government are crucial to these efforts.

 

PCDC is working with its partners to help make these changes happen. We are helping providers build new facilities, transform into medical homes, connect with each other through health information exchanges, and adopt other initiatives that move us toward a more integrated, accountable and patient-centered healthcare system.

 

We look forward to working with our partners to build sustainable, lasting change that will truly transform health care delivery.

New Markets Financing Helps Children's Aid Society Expand Health Services to Bronx Foster Children

The Children's Aid Society (CAS) has assisted foster children for more than 150 years, providing everything from dental care for 7-year-olds who've never seen a dentist to counseling for children removed from their parents' homes. Most of this care is scattered across the NYC, offered in school clinics and care facilities in Manhattan and the Bronx.  

 

However, in 2012, thanks to $7 million in PCDC New Markets financing, more than 4,000 children will be able to receive primary care services at a new Bronx clinic offering comprehensive programs. The 10,000-square foot CAS Bronx Children's Health Care Facility is expected to provide over 15,000 medical, dental and mental health patient visits per year.

The Medical Home Experience Demystified

  

Why is the Patient-Centered Medical Home important to America's health insurers?  This topic was featured at the 15th Annual Willis Health Care Practice Forum, hosted by the Willis Group, global insurance broker and risk advisor. The forum explored the impact of health care changes within the health care professional insurance marketplace.  PCDC's Ronda Kotelchuck and Peter Cucchiara, along with  Donald Lurye, MD, CEO of Elmhurst Clinic, spoke on Medical Home's impact on providers in a presentation titled: "Medical Home Rollout: 2011 and Beyond."

 

PCDC and the Willis Group also partnered to capture the patient's experience "with" and "without" a medical home in the videos featured below.

PCDC Partners with L.A. Care to Deliver PCMH-Based Marketing and Customer Service Solutions

The largest public health plan in the U.S. with over 750,000 Medicaid enrollees, L.A. Care Health Plan and PCDC partnered to complete their "Attracting and Retaining Patients" initiative, which supported five Los Angeles safety net clinics in their marketing and customer service efforts.

 

Aligning with the medical home customer service model, participants were trained to develop and execute sustainable marketing and customer service plans. PCDC led the four learning sessions and provided one-on-one coaching, which included topics such as:

 

  • Assessing and analyzing current patient experience
  • Identifying prospective patients through data collection, analysis, and market segmentation
  • Attracting patients by understanding their unique needs and characteristics

RCHN Foundation Launches New Website "Chronicling" the CHC Movement

Learn about the history of CHCs and more from CHroniCles - a special joint project of the RCHN Community Health Foundation and the Geiger Gibson Program in Community Health Policy.  The multi-media website uses photos, documents and "virtual visits" to capture the amazing history behind the CHC movement, its leaders and its impact on the health care system.

 

New Report from NACHC Demonstrates Impact of Budget Cuts on CHCs 

nachc reportA new report from the National Association of Community Health Centers (NACHC) titled "Access Endangered: Profiles of the Medically Disenfranchised," shows that U.S. families are increasingly turning to health centers for services. Among the report's findings: 

  • For every $1M in federal funding cuts, CHCs lose capacity to serve 8,297 patients.  
  • One in four low-income and uninsured working adults relies on health centers for care.  
  • The number of uninsured patients at health centers grew by 36 percent nationally from 2004-2009, yet state funding to health centers has decreased by 42 percent in the last two years.

New York State Primary Care Policy Update

 

"Redesigning" Brooklyn's Healthcare SystemWith several large institutions in precarious financial shape, The Brooklyn Work Group of New York's Medicaid Redesign Team is redesigning Brooklyn's healthcare system to be more affordable and efficient. PCDC is calling for major expansion and strengthening of primary care infrastructure as part of the redesign. Click here for PCDC's testimony at a recent public hearing.

 

Rapid Expansion of Health HomesThe Department of Health is now accepting applications to provide "Health Home" services, which support coordinated, comprehensive medical and behavioral health care to patients with chronic conditions. Health Homes are eligible for a 90% federal Medicaid match for two years.  The deadline is October 3, 2011. 

 

$325 Million for NY Hospital-Based Medical Homes: Through an agreement with the Centers for Medicare and Medicaid Services (CMS), the NYS Department of Health will receive $325 million for th
e Hospital Medical Home project. Funding will provide incentives to transform primary care teaching programs at hospital and community sites.
 

   

Funding support CHCs and School Based Health Centers:  The Health Resources and Services Administration (HRSA) awarded funds nationwide to create "new access points" for community health centers and expand capacity of school-based health clinics.  Over $12M went to 37 New York school-based health clinics, an investment that is expected to increase capacity by over 50%.  Eight NY health centers  received a total of $2.6 million.

Karla Silverman, Senior Program Manager, Performance Improvement, previously served as Director of Clinical Services at Planned Parenthood Hudson Peconic where she oversaw the delivery of family planning, gynecological and prenatal services in twelve health centers.

 

Jennifer Chen, Development & Communications Associate helped direct strategic fundraising campaigns for Koszyn & Company, a fundraising consultant, and also worked for the Council on Foundations.

On June 13th, leaders from the urban planning, housing, government and health sectors came together at the  Healthy Communities: New York City Conference to explore how the health and community development sectors can collaborate and promote better health outcomes for low income people and communities .

 

The event was one in a series of national meetings supported by the Robert Wood Johnson Foundation, and co-sponsored by the Federal Reserve Banks of New York and San Francisco, the New York Academy of Medicine, and PCDC.
