The heat is on- and I'm not just talking about the weather! After celebrating the landmark victory of healthcare reform this spring, we are now faced with the critical stage of implementation. It is a time that will challenge all of us to re-prioritize and take necessary steps to build a foundation of strong primary care that will reduce costs, improve quality and reduce disparities that have for too long made us a nation of health care haves and have-nots. As we see it, there are two major tasks that we must undertake. First, is the expansion of primary care capacity, so everyone has access to services - not just health insurance. Second, is transforming the model of care into one that is integrated and patient-centered, so we can achieve the quality improvements and cost savings essential to meaningful reform. We look forward to working with our many partners to make this expansion and transformation a reality. In this issue, you will read about how our Capital Financing program is adding primary care capacity throughout the state, as we kick off deployment of our $40 million New Markets Loan Fund. And how we are helping providers in New York and Los Angeles navigate and take full advantage of new opportunities, including patient-centered medical home recognition and meaningful use of health information technology. | Low-Cost Financing Helps Hudson River HealthCare Expand Monticello Site | "This community has long needed additional access to quality health care, but with low-cost financing from PCDC, we are able to establish a major health center in Downtown Monticello, and provide necessary primary and dental care to thousands of patients in the community." -Anne Kauffman Nolon, President & CEO, Hudson River Healthcare
On July 6th, PCDC announced deployment of $2.85 million in financing to Hudson River HealthCare (HRHC), signifying the first project for PCDC's statewide $40 million New Markets Loan Fund. The low-interest, long-term financing will allow HRHC to acquire a prime downtown Monticello property to expand primary care and dental services to low-income patients in Monticello and surrounding communities.
The 25-year loan carries a fixed interest rate of 4.77%, and the loan is structured so that up to 26% of the principal amount may be forgiven. PCDC's New Markets Loan Program, operated in conjunction with HSBC Bank USA, N.A., offers below-market, long-term, fixed rate loans to expand primary care in low-income communities. Additional funding was provided through a $200,000 federal grant secured by Congressman Maurice Hinchey (D-NY).

| PCDC, NYC Organizations Join National Initiative to Improve Access to HIV/AIDS Care | |
"Properly managed, people living with HIV/AIDS can now live healthy, productive lives; but lack of outreach, follow up, and care coordination means many vulnerable New Yorkers are not getting the treatment they need, and the results are often fatal." - Len McNally, program director, New York Community Trust, and Chairman, NYC AIDS Fund.The New York City AIDS Fund of the New York Community Trust has received a $1.7 million grant to bring coordinated and consistent primary care to more than 1,500 New Yorkers living with HIV/AIDS. The partnership called ACCESS NY, brings together PCDC, the NYC AIDS Fund; Amida Care, and local HIV/AIDS specialty provider groups. The partnership will leverage Amida Care's experience in recruiting, enrolling, and maintaining new HIV+ patients, their extensive HIV primary care provider network, and PCDC's expertise in redesigning systems of care to focus on quality, continuity, and patient access. New York is one of five regions nationally to receive the award, which is provided by the National AIDS Fund and Bristol Myers-Squibb.

 PCDC and the Community Health Care Association of New York State (CHCANYS) are launching a new learning collaborative that simultaneously prepares primary care centers for two major upcoming initiatives which link financial incentives to improved patient care. They include NYS Medicaid's medical home reimbursement enhancements and the federal government's payments for "meaningful use" of health information technology. This six-month collaborative program will equip health centers with the knowledge, tools, and resources to: compile a medical home survey submission to the NCQA; collect 90 days of data required for Stage 1 meaningful use objectives; and identify future areas of improvement.
PCDC and CHCANYS will host an information session on July 21st at Baruch College for health centers interested in participating in the Learning Series.  After four years without a permanent administrator and in anticipation of major health care reform implementation, President Obama bypassed Congress last week and appointed Dr. Donald M. Berwick to lead the Center for Medicaid and Medicare Services (CMS); a $700 billion a year federal agency. Berwick is President & Founder of the Institute for Health Care Improvement (IHI), a pediatrician, and health policy expert, dedicated to improving the quality of healthcare in the United States. "Dr. Berwick's leadership will be critical to the expansion and transformation of primary care in the U.S., which is the foundation for the quality improvements and cost savings we expect from health care reform," wrote PCDC's Ronda Kotelchuck in a letter to the members of the Senate Finance Committee. The principles of Dr. Berwick's work in healthcare and clinical practice form the basis for PCDC's Performance Improvement programs, particularly in introducing new knowledge, improving performance, and expanding access to care. Using IHI's theories and methods, PCDC has coached more than 200 primary care teams at community health centers, ambulatory clinics, and hospitals to reduce no-shows, appointment backlogs and cycle times, while improving patient and provider satisfaction. | PCDC Brings Marketing and Community Outreach Strategies to Los Angeles Safety-Net Clinics |
 PCDC recently partnered with L.A. Care Health Plan, the nation's largest public health plan, to help Los Angeles clinics implement marketing strategies to bolster community outreach efforts and ultimately recruit additional patients. Guided by the strategies and tactics of PCDC's Practice Redesign program, the "Attracting and Retaining Patients" will help participating clinics learn the fundamentals of customer service, identify and communicate areas of improvement and better understand the local community's health needs. The nine-month program will focus on delivering customer service that specifically aligns with the principles of the patient-centered medical home and improves patient experience. On June 17th, PCDC's Annual Spring Gala honored four healthcare leaders and welcomed special guests, Mayor Bloomberg and Ted Kennedy, Jr.
More than 450 friends and guests joined PCDC at Bridgewaters in South Street Seaport to celebrate the new prominence of primary care in healthcare reform. The event raised funds to support PCDC's programs and services to help improve access to primary care in low-income communities in New York and across the country.

Kennedy Remembers His Father's Lasting Contribution to Health Center Movement

At PCDC's Annual Spring Gala, in a special keynote address, Ted Kennedy, Jr. delivered a moving tribute to the work of his father, the late Senator Edward Kennedy, acknowledging his role in promoting community health centers when he first came to the Senate.

PCDC and Community Leaders Develop Emergency Response Plan
|
On June 2nd PCDC brought together dozens of health professionals, community leaders, and area residents from Manhattan's Community District 5 to draft a community-wide emergency preparedness plan. Included in the district-wide plan are ten initiatives to be carried out over the next year, including conducting training and drills, and encouraging neighboring institutions to adopt emergency command systems.
Supported by the New York City Council, the program teaches organizations important skills, such as train volunteers and install telecommunications systems, how to dispense vaccines during a patient surge, use phone trees to mobilize their off-duty health care professionals, create systems that alert and update other centers to disasters.

NYS Medicaid Medical Home Reimbursements Get Thumbs Up from Feds
|
After months of anticipation, July 1st marked the official launch of New York State Medicaid's reimbursement enhancements for providers recognized as medical homes by the National Committee for Quality Assurance. Private practices and community health centers are all eligible for enhancements, which are based on NCQA recognition levels and range from $2 to $6 per visit.
Providers seeking NCQA Medical Home recognition should visit PCDC's Medical Home Resource Center, which includes helpful links and fact sheets, PCDC's latest publication Obtaining Patient-Centered Medical Home Recognition: A How-To Manual," and more information on PCDC's medical home services.

| "Money Driven Medicine" Author Screens Film at PCDC | On June 7th, Maggie Mahar visited the PCDC staff to screen the documentary film, Money-Driven Medicine, based on her 2006 book Money-Driven Medicine: The Real Reason Health Care Costs So Much. The film provides an essential introduction to the U.S. healthcare system and the economics that drive it. The film includes stories from doctors and patients, plus insights from Dr. Donald Berwick, Dr. James Weinstein of Dartmouth and bio-ethicist Dr. Larry Churchill of Vanderbilt.
Ms. Mahar is a fellow at The Century Foundation and writes the "HealthBeat Blog," author of Bull! A History of the Boom, 1982-1999 and a former financial reporter for Barron's, Money Magazine, The New York Times, and Bloomberg.
