New CHAS Questionnaire

CHAS have now released the new Appendix 2 Questionnaire for 5 or more employees , (including CDM-C and Designer) - the changes have been made to make the form more user friendly.

The old version (7.5) appendix 2 will only be accepted until 1st November 2010.
