About Our Practice
While no one likes going to the doctor we are dedicated to providing the best clinical experience a medical practice can offer. Our core practice is Internal Medicine.
We are all going to age, thus our philosophy is to provide services that help prevent disease as well as treat them. We want our patients to live long and healthy lives, feel good about themselves and enjoy life. By incorporating lifestyle management strategies such as nutritional counseling we can our patients achieve these goals.
While not a medi spa we do provide cosmetic skincare for patients who desire these services. We believe all of our patients are beautiful just the way they are. If you would like to enhance your appearance we provide a full range of cosmetic skincare products and procedures. All of our injectable procedures are performed by Dr. Bernstein. We are committed to obtaining optimal results in a cost effective manner.
Dr. Bernstein has an extensive background in pulmonary diseases where his major area of interest was asthma and exercise physiology. Twenty-five percent of the population suffer from allergies. We are uniquely qualified to provide asthma and allergy services to our patients because of Dr. Berrnstein's pulmonary background.
Most importantly we want to provide an environment that makes our patients feel comfortable. We view our patients as an extension of our family. That is why there will always be someone to answer the phone, fit in emergency visits for same day appointments and we still make house calls.

Highlights of Healthy Aging Seminar and Workshop (part 2)
In this part of the lecture,We focused on more practical aspects of diet. We base our recommendations on a low glycemic Mediterranean diet because this diet has been associated with decreased mortality from all diseases. Focus should be placed on consumption of vegetables and fruits.with a goal of consuming 9 vegetables, legumes and fruits a day. We recommend 6 vegetables and 2- 3 fruits. Fruits should be low glycemic such as berries and apples. Pineapple, raisins, dates and bananas are higher glycemic and should be consumed less frequently. Fish is the primary source of animal protein,along with small amounts of eggs,and poultry. Red meat should be consumed in small amounts and preferably in the form of lean cuts such as flank steak and sirloin. Nuts and seeds are emphasized but only in small amounts as they are extremely calorie dense.Whole grains and dairy products are recommended but in limited amounts. Olive oil is recommended as a source of fat along with nuts and seeds. Specific recommendations can be obtained from our office with our dietary handout
Weight loss can be a very trying experience for some.
Here are some helpful tips for success.
Eat the correct foods,
Eat foods in moderation,
Focus on stress to avoid binge eating,
Include Regular exercise as part of your regimen.
An essential part of a weight-loss program is keeping a dietary log.When I ask people "How is Your diet?" the universal answer is "Good". When I see people for nutritional counseling, I always recommend they keep a diet log. Most people eat more than they think they do. Keeping a diet log, is a great way to help us learn about portion control as well as about making the proper choices with the foods we eat. The one caveat to this is don't beat yourself up mentally if you are not making all of the correct choices. You are merely recording information. People do better over time if they make gradual changes that become permanent lifestyle changes instead of trying to lose weight all at once.
I do recommend some supplements but remember they are no subsitute for a healthy diet. In particular,There is increasing evidence to suggest that daily supplementation with 1000 to 2000 iu of vitamin D can be helpful. Low vitamin D levels are associated with depression, colon cancer, prostate cancer, breast cancer and of course Osteoporosis. .In patients with metabolic syndrome,
fish oil supplements can lower triglycerides.A vitamin B complex is good protection against some of the inherited disorders of homocysteine metabolism. However excessively high doses of folic acid (5mg) have been associated with an increased incidence of colonic polyps and should be avoided. There's some evidence to suggest that selenium may be protective in helping to prevent prostate cancer.

Tips for a healthy Summer
Beach Tips:
1. Use sunblock. Spf of 30 is adequate as it provides 98 % protection. If you use a conventional sunblock you need to apply it 15 - 30 minutes before going out in the sun so it can activate. These sun blocks require absorption by the skin to be effective. If you use a reflective block [ sun blocks that contain zinc oxide like skinceuticals] you get immediate protection and the added benefit of long lasting UVA protection to prevent wrinkles. All sunblocks require reapplication every few hours.
2. Stay well hydrated.
3. Jelly fish stings can be a nuisance. Keep a small bottle of vinegar in your beach bag. If stung apply immediately. This will help prevent imbedded tentacles from firing. Than apply hot water to the wound. This is the most effective treatment for pain.
Tick bites:
1.Keep dogs and cats free of ticks.
2. If imbedded ticks are removed and treated within 72 hours with 200 mg of doxycycline the risk of developing lyme disease approaches zero.
Poison Ivy:
1.If exposed remove all clothing and wash in hot
water
2. Rinse off all exposed areas with lukewarm water, then soap up and rinse again.
Allergy:
Dampness and heat are a great recipe for mold growth. We recommend that you run a dehumidifier 24 hours a day. For basement storage, plastic container should be used and cardboard containers should be thrown out.
Heat related illness:
Playing sports on a hot day increases the risk of heat related illness. Start loading up on fluids before you go outside. If you have heat related symptoms such as headache or dizziness, stop playing and get to a cool place. More severe symptoms such as nausea and vomiting, cramps, or altered mental status require medical attention. Avoid muscle building supplements such as creatine which may increase the risk of heat related illness.
The primary therapy for heat related illness is rapid cooling and removal from a hot environment. The aggressiveness of therapy is indicated by the severity of symptoms. The above recommendations are only a guide and not a substitute for medical advice as individual circumstances vary.. If you think you may have heat related illness and have concerns, contact our office or seek medical help immediately.
Water damage:
With hot summer weather, thunderstorms are always a threat. If you inadvertently keep your windows and sunroof open, this can cause a mess. Don't despair. The first step is to vacuum everything you can with a wet dry vacuum cleaner.This will get moisture out of carpeting and upholstery initially.Then place a dehumidifier (remember to empty frequently) in the car, and run for 24 to 48 hours.This will remove all remaining moisture, and prevent subsequent mold growth.

Lessons from Avandia
As many of you know now a recent article in the New England Journal of Medicine highlighted the increased risk of heart disease associated diabetes drug Avandia. The incidence of heart attack in patient's who took the drug was 1.6 out of 100,versus 1/100 in patients who did not take the drug. In absolute risk terms this is a relatively small amount. However the whole purpose of treating diabetes is to prevent secondary complications. Secondary complications of diabetes include heart disease, peripheral vascular disease, blindness, and kidney disease. Avandia was a drug that treated insulin resistance. Theoretically the drug made lots of sense . Insulin resistance is the primary cause of Diabetes. However there were unforeseen complications from taking this medication. Because Avandia is associated with an increased risk of heart disease,the benefits of this medication seem to outweigh the risks..
The lesson from this In my view,is that our primary mode of treatment should always be lifestyle modification. As we discussed in last month's newsletter there is evidence that lifestyle modification can be more effective than medications in preventing diabetes, and there is evidence lifestyle modification is associated with a decreased mottality risk from all diseases by up to 50%. .Drugs can be life saving and are a very important part of treatment strategies to address disease. However we should never lose site of the fact that the use of medications can have unforeseen consequences. All medications (including nonprescription medications, herbal supplements and some vitamins) have risks as well as potential benefits.
