 DOPPS Kidney Week Symposium Informs Dialysis Practice

 

For the twelfth consecutive year, DOPPS will present a symposium at ASN's Kidney Week. The presentation is entitled "International Variability Informs Best Hemodialysis Practices: The Dialysis Outcomes and Practice Patterns Study (DOPPS)." The program will present the latest practice trends in the DOPPS Practice Monitor, current therapies for mineral and bone disorder, and international vascular access practices, among other topics.

dialysisNew Findings: High Ultrafiltration Rates Associated with Higher Mortality

 

Arbor Research investigators examined the risks of high ultrafiltration rates in analyses that will be presented at ASN Kidney Week. Along with the University of Michigan Kidney Epidemiology and Cost Center (UM-KECC), Arbor Research conducted these analyses under the Centers for Medicare & Medicaid Services (CMS) ESRD Quality Measure Development and Maintenance contract. This new research uses CROWNWeb data to analyze death rates among all patients at a facility, categorizing facilities by their ultrafiltration practice pattern.

 

The reasons for prescribing high ultrafiltration and approaches to reducing/optimizing ultrafiltration requirements in dialysis patients deserve urgent attention by the dialysis community. Death rates could plausibly be lowered by 4%-7% at the 40% of facilities with the highest percentage of patients with ultrafiltration rates greater than 15 ml/kg/hr if those facilities adopt the practices of the facilities with the lowest rates.

Quality Incentive Program (QIP) Associations with Mortality

  

The Centers for Medicare & Medicaid Services (CMS) implemented a Quality Incentive Program (QIP) for payments to dialysis facilities, beginning with payments made on January 1, 2012. Researchers at Arbor Research and the University of Michigan Kidney Epidemiology and Cost Center (UM-KECC) used 2009 CMS claims data to examine the association between the individual QIP measures for Payment Year 2012, and the CMS facility standardized mortality ratio (SMR). The SMR is one of the quality measures reported by CMS on Dialysis Facility Compare. The results indicated that facilities receiving payment reductions based on their total QIP score had higher mortality than facilities with no payment reductions.
