- 24 hour and delivery exposures evaluated on individual basis
Submission requirements:
- ACORD 130 Workers' Compensation Application
- 4 prior years historical aggregate payrolls by year
- 4 years loss runs including current year, valued within 90 days
- Financial information for premiums greater than $350,000
- New ventures - resume required
