The Final Steps Toward MDS 3.0

Friday, October 1, 2010 is only three days away.  SNF's must put the final touches on their transition plan to implement MDS 3.0. 

An MDS 3.0 readiness checklist:

  • Review the entire Medicare Part A caseload and choose which CMS Transition Ooption best suits each claim.
  • MDS and Rehab set the ARD for each transition MDS and communicate the ARD and due date to the interdisciplinary team.
  • MDS, Rehab and the business office must do a triple check of the transition assessments to confirm the ARD, RUG score, MDS type and primary ICD-9 codes.
  • Interviewing staff should review the Appendix D to assure all of the interview strategies are well understood.
  • Confirm who will conduct the staff interview if the patient is unable to participate.
  • Have a MDS 3.0 Toolbox readily available with cue cards, flashlights, newspaper print etc.
  • Confirm that the facility has the most updated version of MDS 3.0 RAI manual, including the September 13, 2010 updates.
  • Provide on-going informational sessions for direct care staff to discuss the critical observations of patient behavior necessary to accurately code each MDS.  Discuss Sections G, C, D, F and J in particular.
  • Develop a task force to implement a "skilled" Restorative Nursing Program if the facility lacks this program.
  • Review, discuss and train all nursing staff on documentation techniques who will be responsible for daily skilled documentation of wounds and skin integrity issues.
