MDS 3.0 Section J: Defining Falls
What the MDS 3.0 RAI manual now says about falls:
Falls - The RAI manual defines a fall as "an unintentional change in position coming to rest on the ground, floor or onto the next lower surface (e.g., onto a bed, chair, or bedside mat). The fall may be witnessed, reported by the resident or an observer or identified when a resident is found on the floor or ground. Falls include any fall, no matter whether it occurred at home, while out in the community, in an acute hospital or a nursing home. Falls are not a result of an overwhelming external force (e.g., a resident pushes another resident).This section of the MDS addresses fall history upon admission (180 day look-back), falls since admission / prior assessment, the number of falls which occur since admission / prior assessment and if any injury minor or major was acquired as a result of the fall. A fall should stimulate evaluation of the resident's need for rehabilitation or ambulation aids and of the need for monitoring or modification of the physical environment. Falls are a leading cause of morbidity and mortality among nursing home residents and can result in serious injury, especially hip fractures. Previous falls, especially recurrent falls and falls with injury, are the most important predictor of future falls and injurious falls. Identification of residents who are at high risk of falling is a top priority for care planning."
The facility will be responsible for collecting data regarding a history of falls for new admissions dating back 6 months. Section J also collects data on the occurrence and number of falls since admission or most recent assessment and whether injury was sustained. Harmony recommends that the interdisciplinary team discuss the manual's definition of falls. A facility policy regarding notification of the proper staff member when a fall occurs should be in place come October 1, 2010. Fall prevention programs will likely continue to be a significant area of focus for state surveyors.
